目的:为了评价Setffee钢板内固定加植骨对下腰滑脱症的疗效。方法:运用Setffee钢板内固定加椎体间植骨融合术治疗下腰椎滑脱症病人38例。结果:系统随访4个月-3年,全组病人滑脱均有不同程度的改善:Ⅰ度滑脱平均复位率90.2%,Ⅱ度滑脱平均复位率84%,Ⅲ度滑脱平均复位率70.0%,骨性融合率为90%。结论:Setffee钢板内固定给不稳的腰椎以支持与固定,有利于植骨的愈合,术中减压解除了狭窄,有利于改善病情。  相似文献

目的:分析腰椎间盘突出症术后对侧复发的原因、手术指征.评价翻修手术的效果和预防。方法:分析1993-2004年对8例对侧腰椎间盘突出症复发的患者进行再手术治疗的临床资料及手术效果。结果:经平均16个月的随访.按改良Macnab疗效评定标准.优5例.良2例,可1例,优良率87.5%。结论:再手术的主要原因是初次手术摘除髓核不彻底等因素造成。预防措施为采用对脊柱稳定性破坏小的术式。手术时尽量将髓核取尽。  相似文献

目的:探讨横突间植骨融合内固定与追歼植骨融合内固定对腰椎滑脱症的临床治疗效果.方法:选择我院2012年3月至2014年5月收治的86例腰椎滑脱症患者为研究对象,随机分为A组43例,行横突间植骨内固定术治疗,B组43例,行椎间植骨融合内固定术治疗,对两组患者治疗后疗效及症状改善情况作对比分析.结果:两组患者手术时间与术中出血量对比无明显差异(P0.05);术后住院时间对比显示,B组显著优于A组,差异有统计学意义(P0.05);两组患者融合率与治疗后并发症发生率对比显示,两组无明显差异(P0.05);观察组治疗优良率为93.02%,对照组为69.77%,差异有统计学意义(P0.05).结论:椎间植骨融合与横突间植骨融合术对腰椎滑脱症进行治疗的手术相关指标及并发症发生率、融合率等无明显差异,在I度退变性滑脱治疗中,横突间植骨是较为有效的治疗方案;真性活脱与I度以上滑脱行椎间融合术治疗,疗效更佳.  相似文献

目的:探讨老年人大肠癌的外科治疗方法。方法:回顾分析116例老年大肠癌的临床资料。结果:误诊率35%,共存病62%,肿瘤切除率75.9%,术后并发症发生率28%,围手术期死亡率5.7%。结论:手术切除是老年大肠癌病人的最好治疗方法,围手术期的正确处理是提高疗效的关键。  相似文献

腰椎滑脱症是常见的脊柱疾患,指椎体前移造成椎管狭窄,致硬膜囊、神经根受压、牵伸而出现的一系列神经系统症状,是慢性腰腿痛的常见原因之一。有效的减压、复位、矫形固定、椎间融合技术,是症状缓解和维持远期疗效的关键。近年来扩大了手术的适应征,手术治疗比非手术治疗疗程短、痛苦小,往往立竿见影,本文对34例外伤性腰椎滑脱分析总结,对腰椎滑脱的若干问题进行了讨论。  相似文献

目的:通过27例高位腰椎间盘突出症的回顾性研究,提高对本症的认识,减少漏诊、误诊。方法:回顾性分析高位腰椎间盘突出症患者27例的临床和手术资料。结果:27例患者均采用手术治疗,术后对21例进行了0.5~6年(平均2,5年)的随访,结果优12例,良6例,可2例,差1例,手术优良率85.7%。重点讨论了高位腰椎及椎间盘和神经根的解剖特点和临床特征及诊断治疗。结论:对不同程度的患者需用不同治疗方式。  相似文献

目的:探讨改良的额肌瓣悬吊术治疗重度先天性上睑下垂的临床疗效及影响疗效的因素。方法:对80例(114眼)确诊为重度先天性上睑下垂的患者实施改良的额肌瓣悬吊术,记录并分析其临床资料,进行疗效评估。结果:114眼中矫正满意者106眼(93%),欠矫者5眼(4.4%),过矫者3眼(2.6%),回退者7眼(6.1%)。结论:改良的额肌瓣悬吊术治疗重度先天性上睑下垂疗效肯定,是目前治疗重度先天性上睑下垂的首选术式。  相似文献

目的:探讨改良去大骨瓣减压术在临床上治疗重型颅脑损伤的疗效.方法:回顾性分析我科自2010年7月~2012年10月以来52例重型颅脑损伤患者采用单侧改良去大骨瓣减压术的预后及其并发症.结果:术后恢复良好者19例,中残11例,重残5例,植物人生存4例,死亡13例,总预后不良率42.31%.结论:改良去大骨瓣减压术具有骨窗范围大。脑挫裂伤灶显露好,更有利于清除颅内血肿及损伤的大脑半球、侧裂血管的减压,能有效降低颅内压、解除脑疝,从而达到改善预后、减少并发症、提高病人生存质量的目的.在临床急重型颅脑损伤患者的治疗中应用广泛,疗效确切.  相似文献

目的:观察RF-Ⅱ型椎弓根螺钉系统加横突间植骨治疗腰椎滑脱的疗效.方法:自1998年2月~2001年2月采用RF-Ⅱ型椎弓根螺钉系统复位内固定加横突问植骨术治疗腰椎滑脱症21例,男性15例,女性6例,峡部型17例,退变型3例,创伤型1例,Ⅰ度5例,Ⅱ度12例,Ⅲ度4例,滑脱部位L515例,L46例.结果:全部病例均获随访平均3.2年(2.4~5年),复位率90%~100%,无感染,无神经根损伤或神经症状加重,术前畸形得以改善,19例术前症状消失,20例(95%)4~6个月脊柱融合.结论:RF-Ⅱ系统强度可靠,三维固定方式理想,可明显提高横突间植骨的融合率,是目前治疗腰椎滑脱的理想方式.  相似文献

During interview with English majors, it was found that a majority of students expressed disappointment with their experience of English literature classes. Students' dislike of learning English literature appears to be the major problem of teaching English literature in China. Reader response approach is a good way to solve this problem. In this paper, I explain the rationale behind the reader response approach, such as its origin, definition, features, assumptions, and strategies. Then I illustrate how reader response approach works as a teaching strategy by presenting several teaching models.
