辩证行为疗法(dialectical behavior therapy,DBT)是在传统认知行为疗法基础上发展而来的一种新型认知行为心理治疗方法,由美国华盛顿州立大学教授玛莎·莱恩汉(Mar-sha M Linehan)博士于20世纪90年代初创立,它以辩证法的基本原则为理论基础,强调病患与治疗师之间,理性与感性之间,接受与改变之间的辩证训练和策略的应用。该疗法的治疗策略主要包括辩证法策略、合理化认同策略和啦啦队鼓励策略等,辩证法的策略贯穿于整个治疗过程。文章以一边缘人格障碍患者为例,具体说明辩证行为治疗策略的应用。 相似文献

College counseling centers report increased student presentation with severe psychological issues (Gallagher, 2012 ). Although dialectical behavior therapy (DBT) has demonstrated efficacy with multiple clinical populations, standard model DBT is not feasible for many traditional college counseling centers. This article describes the iterative development and evolution of a comprehensive DBT program adapted to fit the scope of a traditional college counseling center. Abstract Most college counseling centers have adopted session limits for their psychotherapy clients though little evidence exists to support such cutoff points. In the past eight years, a few studies have been published using survival analysis statistics for investigating the speed of recovery in psychotherapy. The outcome of clients at a university counseling center was studied to assess change across time, on a session-by-session basis. The analysis of these data showed that 14 sessions of psychotherapy were required for 51% of clients to meet criteria for clinically significant change. Results were interpreted as suggesting that limiting psychotherapy at college counseling centers may result in improvements that are not clinically meaningful for the majority of clients receiving services. 相似文献

Abstract The current study assessed how previous experience with therapy impacted college students' perception of a clinician displaying symptoms of burnout. One hundred forty-seven undergraduate students viewed a videotaped therapy vignette depicting segments of three consecutive psychotherapy sessions. The therapist displayed symptoms of burnout (e.g., fatigue, clock watching) in one of the sessions. Participants rated the therapist on the following variables: liking, competence, level of concern, dedication to the field, and whether the client should continue to be treated by that therapist. As predicted, participants who indicated that they had been in therapy in the past rated the therapist significantly lower on four of these dimensions compared with subjects who had no prior experience in therapy. The results suggest that students who have previously been in therapy are more critical of a fatigued and distracted therapist. 相似文献

This study provides information about students seeking counseling (N = 3,844) at 9 institutions of higher education. The K-PIRS, an empirically validated measure, was used to assess 7 problem areas (mood difficulties, learning problems, food concerns, interpersonal conflicts, career uncertainties, self-harm indicators, and addiction issues). Forty-two percent of students presented with multiple problems, and most reported that their concerns interfered with their academic (87%) and social (90%) functioning. A majority of students (61%) were in a stage of contemplation when seeking counseling. Only 24% were in a stage of action. There were small differences in problem scores by participants' gender, ethnicity, year in school, type of residence, work status, previous treatment, and use of psychiatric medication. Implications are discussed for counseling practitioners working with college students. 相似文献

This pilot project investigates the efficacy of an 11-week dialectical behavior therapy skills training class in increasing adaptive skill use and emotion regulation in college students with cluster B personality disorders or traits. Subjects are students (N?=?19, mean age = 21.31, 1 male, 18 female) at a southeastern university counseling center. Students meeting criteria but not interested in attending the group were recruited to serve as control subjects who received treatment as usual. As compared to treatment as usual, dialectical behavior therapy participants had significant improvement in increasing skills use and decreasing maladaptive coping skills; there was a trend for improvement in difficulty regulating emotions. 相似文献

With an increased demand for mental health services, perceived increased severity of students presenting for services, and increased number of students prescribed psychotropic medications, university and college counseling center directors have had to increasingly identify psychiatric service options. Psychiatry residents can potentially help fill these needs. To better understand the role of psychiatry residents in university and college counseling programs, we conducted a web-based survey of United States psychiatry residency program directors. This study describes clinical activities and rotation characteristics of psychiatric residents working in counseling centers. Of the 48 residency programs that responded, 27 provide rotations at university or college counseling centers, and of those with rotations, 100% report that the residents rate these as very good or excellent. Expanding psychiatry residency rotations at college counseling centers may be mutually beneficial both to the centers and to psychiatric residents. 相似文献

The assessment and treatment of students with borderline personality traits or disorders poses specific challenges for clinicians at college and university mental health centers complicated, frequently, by a model of brief treatment frameworks and high patient volumes. Students with borderline pathology will be well represented in student health centers and will likely generate unusually complex and concerning situations for clinicians and administrators. Accurate diagnosis of borderline pathology, avoidance of iatrogenic complications related to misdiagnosis, and referral to informed and evidence-based interventions can guide clinicians’ management. Risk management concerns are best addressed by a clear and realistic understanding of the borderline pathology. The author proposes a variety of interventions for college and university mental health clinicians with the goal of accurate diagnosis of borderline personality, management focusing on safety for both students and clinicians, and targeted referrals for both borderline symptomatology and co-occurring conditions. 相似文献

根据自愿报名选取大一12名学生作为研究对象,实施3周共6次团体辅导,探讨在大学生职业生涯规划教育中团体辅导的方式及效果。用《团体单元回馈表》、《团体成员回馈表》和《大学生职业生涯发展问卷》对团体辅导进行评估。团体成员对辅导干预内容的有效性、团体的满意度等方面都给予肯定。干预后学生在就业信心、职业认知、自我认识、规划认知、总体评价方面有显著提高。这说明团体辅导对大学生职业生涯发展规划教育效果显著。 相似文献

Psychometric evaluation of presenting problem checklists is vital, given increasing clinical severity among college students. However, checklist research has focused on students at public universities and utilized inappropriate methodologies when doing so. It is unclear whether checklists used at academically selective universities reliably and comprehensively assess the potentially unique presenting problems of their students. Exploratory and confirmatory factor analyses with independent samples examined the checklist used at an academically selective university's counseling center. A six-factor model best fit the data, suggesting that this checklist reliably and comprehensively assesses presenting problems at this university. This model holds detailed assessment, outreach, and policy implications for counseling centers. 相似文献
