Coccidioidomycosis is an endemic disease of the Western hemisphere. The cases in non-endemic areas are mostly imported. Determining
a history of exposure is critical for performing the diagnosis of coccidioidomycosis, especially for cases occurring in a
non-endemic area. In this study, a 71-year-old Chinese male presented to our hospital with chronic cough and malaise, and
was found to have a mass in the middle lobe of right lung. He had been visiting Arizona, USA for four months before admission.
Pulmonary coccidioidomycosis was confirmed by the results of histopathological examination after lobectomy. Typical microscopic
features of Coccidioides are definitely diagnostic, but need be differentiated from other budding yeast infection or carcinoma histopathologically. 相似文献

Chylothorax is an uncommon disease where fatty fluid accumulates within the chest cavity. Conservative manage- ment, including repeated thoracentesis or pleurodesis, seems to be suitable to most cases. Herein, we present a case of efficacious pleurodesis by intrapleural injection of Sapylin, a streptococcus preparation, for the treatment of chylothorax. A 52-year-old non-smoking female farmer was diagnosed as idiopathic chylothorax after we ruled out possible causes including chest trauma, lymphoma, lung cancer, filariasis, tuberculosis, and etc. Two-time intra-thoracic injection of 3 Klinische Einheit (KE) Sapylin achieved rapid and effective control of chylothorax with no severe side effects. Sapylin may facilitate pleurodesis by producing a strong inflammatory response. 相似文献

Adenomas of the nonpigmented ciliary epithelium (NPCE) are often clinically indistinguishable from amelanotic malignant melanomas of the ciliary body or metastatic carcinomas. This paper reports a case study of a distinctive variant of adenoma of the NPCE, which clinically appears as epiretinal membrane in the macular region. Histopathologic studies have revealed this is an adenoma of the NPCE. Identification of this clinic feature is important because it will miss the diagnosis of the adenoma of the NPCE. In this case study, B-scan ultrasonography as well as computerized tomography (CT) has been used to provide help in diagnosing the ciliary body tumor. Because of their anterior location in the ciliary body, partial lamellar sclerou-vectomy is an effective method of treatment. 相似文献

Rhodococcus equi, previously known as Corynebacterium equi, is one of the most important causes of zoonotic infections in grazing animals. Increased cases of human infection with R. equi have been reported, especially in immunocom-promised patients, within recent years. We present a case of R. equi bacteremia in a 51-year-old man with diabetes and liver cirrhosis, on long-term corticosteroid therapy after skin-grafting surgery. The patient recovered soon after he was treated with vancomycin. This review focuses on the microbiological characteristics of this organism, and the diagnosis and treatment of this infection. 相似文献

Peer evaluation is the process whereby students critique the performances of other students. A peer evaluation format emphasizes skills, encourages involvement, focuses on learning, establishes a reference, promotes excellence, provides increased feedback, fosters attendance, and teaches responsibility. The process of peer evaluation is explained, the criteria are specified, the training for use is described, and how standards are maintained is explained. Finally, objections are suggested and addressed. Peer evaluation is a unique, controlled, and precise form of classroom evaluation that involves carefully constructed evaluation instruments, structured classroom situations, and exact evaluative assessments. Conclusion This paper has described the intertwining of linguistic, cognitive, and emotional development in a boy who has a severe language
disability. Development proceeded through the usual stages and at the expected ages, yet there were interesting shifts and
interactions that appear to be a function of Tony’s individual developmental style, and also, possibly, of intervention. One
is particularly struck by the diminishing significance of language comprehension difficulties; expressive problems continue
to be troublesome, but verbal conceptual ability has developed well and Tony has acquired an enthusiasm and affection for
words. He will continue to struggle with verbal expression, but he knows his difficulty well and has made enough strides in
working with it to hold his own in a relatively demanding private school.
Early identification of the problem and intervention that has spanned nearly seven years have permitted the observation of
developmental regressions and advances which, if viewed statically, might have been misinterpreted. More important, the continued
involvement has made possible an emotional commitment between student and therapist that, one hopes, has had a steadying effect

Fulminant type 1 diabetes is a recently discovered subtype of idiopathic type 1 diabetes,defined as diabetes with an extremely rapid process ofβ-cell destruction and progression to hyperglycemia and ketoacidosis.In this report,we present a case of fulminant type 1 diabetes in a 45-year-old Chinese woman,along with a review of the literature.The patient presented with sudden onset of polydipsia and polyuria after flu-like symptoms.Findings on admission included a high blood glucose level and ketoacidosis,but normal HbA1c level.The C-peptide stimulation test showed severe impairment of insulin secretion.Autoantibodies to glutamic acid decarboxylase(GAD) were negative.These results are compatible with the diagnosis of fulminant type 1 diabetes.Human leukocyte antigen-DR7(HLA-DR7) was available in this case.It is concluded that this rapidly progressing type of diabetes exists,and we propose that HLA-DR7 might be predisposed to fulminant type 1 diabetes in Chinese patients. 相似文献

Actinomycosis is an uncommon disease, which is usually manifested as cervicofacial infection and related to poor oral hygiene or compromised immune function. Pulmonary actinomycosis is rare, but its diagnosis is changing due to its variable presentation and the similarity in appearance to other intrapulmonary diseases. Here we report an 80-year-old man with a solitary pulmonary nodule over the left upper lobe. Pulmonary neoplasm was highly suspected in this patient and thus resection of the mass was undertaken through video-assisted thoracic surgery (VATS). Histopathological examination demonstrated this patient had an Actinomyeces infection. While the application of VATS in patients with pulmonary actinomycosis has rarely been reported in literature, we conclude that VATS is valuable for the diagnosis and treatment of patients with undetermined pulmonary nodule(s). 相似文献

Chinese international students make up the largest number of international students in the United States. They face many stressors that may impact their mental health, such as language barriers, social isolation, academic and financial pressures, and acculturation stress. Yet, there is limited guidance on case conceptualization and treatment models for Chinese international students in university counseling center settings. We present a case report of a 19-year-old, first-year, female undergraduate student from China to illustrate a comprehensive approach for the mental health treatment of Chinese international students. Based on our understanding of the interaction between the student and her influencing environment from a multicultural and ecological vantage point, we intervened on a systems level to achieve a favorable outcome. Our interventions included a multidisciplinary team approach with a student-centered perspective, and advocacy on an individual and organizational level. We conclude with suggested guidelines for the treatment of Chinese international students. 相似文献

What learning conditions are necessary to effectively support the knowledge-building enterprise in a learning community? To answer this question, the case-study method was employed to investigate collaborative knowledge building in a graduate level course designed to incorporate specific constructivist learning principles. These principles included (a) having a collective and authentic community goal to facilitate collaboration and engagement in the community, (b) using cooperative groups to provide for social negotiation and to promote multiple perspectives, (c) personally selecting of course readings and group decision-making to encourage ownership in learning, and (d) employing an integrated set of technology tools to archive the work of the community and facilitate communication. Results indicated that the instructional strategies designed into the course promoted collaborative knowledge building and the acquisition of key concepts through self-directed learning strategies. Results also pointed out the types of technological support required to implement an effective community of learners. A proposed set of guidelines is discussed to assist instructional designers in the design and development of collaborative, knowledge-building environments. 相似文献

This study is concerned with possible differences in interaction patterns between teachers and certain groups of students: high and low achievers, and boys and girls. Detailed and comprehensive records of all verbal teacher-student interactions, both public and private, were obtained through videotaping a grade 6 class in three different settings: mathematics, language and science. While there was much consistency in the way the teacher monitored in this study interacted with the different groups, some important differences occurred in the quality and quantity of his interactions with students perceived as best and weakest, and with boys and girls in mathematics classes. 相似文献

This paper reports the development of a progress file, a tool to promote learning, planning and the recording of achievement, for dental therapists in training and professional practice. Progress files are to become obligatory in higher education in the United Kingdom. The aims of the file, its development and the naturalistic approach taken to evaluation are described; the outcomes are analysed. A number of potential challenges posed by progress files for teaching, learning, assessment, curriculum and policy development in the United Kingdom, and for researching these challenges, are highlighted. Issues relating to employment, self-appraisal, lifelong learning and reflection in learning are considered. The desirability of a discipline or professionally-based file, and for acceptance of flexibility in intent, length and usage of such devices is posited. 相似文献

As illustrated in the embedded case study, generically developed models that, in turn, can be adapted to context‐specific applications often have the greatest practical value.
