Directories,Universities, Medical Middleware,and the Real World

Recent developments in high-speed network initiatives have raised awareness of an increased potential for sharing information among institutions. At the core of current plans to take advantage of this potential is the use of standards-based directory servers as repositories of authentication and authorization data. Such repositories can be used to regulate and facilitate access to network resources between persons associated with institutions or with cooperating institutions. Included in these plans is provision for storing and using certificates to establish trust relationships between partners communicating over the network. When the environment is medical, other information such as credentials or access paths to patient data may also be stored in directories. The question has been raised whether, in emergencies, a mechanism for bypassing the more rigorous authentication and/or authorization requirements be provided to allow access to information essential to treating the emergency by individuals who would otherwise not be allowed such access. There is an increasing awareness of the fundamental conflict between a patient's right to privacy and the patient's right to receive medical treatment in a complete and efficient manner.

Medical information, health sciences librarians, and professional liability

As a gatekeeper to medical literature and a critical link in the delivery of information to physicians, the librarian's role raises the issue of the librarian's professional liability. The paper suggests several ways in which liability may attach to the librarian or the librarian's employers. Although the librarian's personal risk is negligible, the physician's exposure due to ineffective library work is substantial since the courts have held that a physician must keep abreast of progress in his field. Librarians can also become associated with professional liability actions as part of a case against a physician or hospital through the legal doctrine of vicarious liability. The paper concludes by suggesting several proactive steps for health sciences librarians to pursue to insulate themselves from professional liability and to insulate physicians and institutions from vicarious liability.

A bibliography of 31 academic journals in medical sociology, medical anthropology, and contemporary social medicine with a focus on public and global health is provided. Along with basic identifying information, the bibliometric measures of Eigenfactor, Article Influence Score, SCImago Journal Rank, and H-index have been specified for each journal when available. Brief annotations along with both Library of Congress and medical subject headings are supplied. Though some limitations are acknowledged, this bibliography serves as a valuable tool for locating and selecting a wide range of periodical scholarship devoted to the social and cultural features of health and medicine.
