From Psychoprophylactic to Orgasmic Birth

The assumptions on which educators based childbirth education principles were valid when psychoprophylactic birth became available. Yet, educators and health-care providers have changed their assumptions about birth as they have learned more from the midwifery model of care, how women want to give birth, and how capable the body is to give birth. Educators'' teaching must now emphasize the synchrony of hormones that facilitates birth, the Sphincter Law, and the sharing of woman-to-woman stories throughout the generations if birth is to be set in its rightful place—in the hands of the mothers.     

Coping With the Unexpected in Childbirth: A Thematic Analysis

This article reports original research that describes new mothers'' experiences of birth and maternity care. Qualitative data were collected through a survey on birth satisfaction, which included space for women to provide comments about their birth and experience of care. Thirty-nine women provided comments that were analyzed using the thematic analysis method. Two themes emerged from the women''s experiences: “Unexpected birth processes: expectations and reality” and “Coping with birth: the role of health-care staff.” Participants described unexpected birthing processes, their experiences of care, and maternity care staff''s contributions to coping with birth. Implications for practice for childbirth professionals include promotion of physiologic birth, respectful person-centered care during all phases of perinatal care, and the value of childbirth preparation.     

Louisa's Birth

In this column, Rachel Mann shares the story of the birth of her third daughter, Louisa. After a previous pregnancy loss, Mann chose to give birth to her third baby in a hospital with attending care from an obstetrician. In spite of the high-risk medical environment, she was able to have an unmedicated, powerful birth. Mann's careful planning, the support of her husband and doula, and her confidence in her ability to give birth helped make Louisa's birth all that Mann hoped it would be.     

Jonah’s Birth

Rachel Goldstein shares her experience of exploring options related to care provider and place of birth early in her pregnancy. Goldstein and her husband, Marc, after reading and research, chose midwifery care and a home birth. She shares the story of a long labor at home supported by her husband, her doula, and her midwife. Her positive attitude, her ability to use various comfort strategies, and the support she received throughout labor contributed to being able to give birth naturally and ecstatically to her son Jonah.     

In This Issue—Reminiscing About Birth and Motherhood

In this column, the editor of The Journal of Perinatal Education reminisces about her births and those of her three daughters and how birth has had an impact on her personal and professional life. The editor also describes the contents of this issue, which offer a broad range of resources, research, and inspiration for childbirth educators in their efforts to promote, support, and protect natural, safe, and healthy birth.     

Robyn’s Birthing Story

This “Celebrate Birth!” column describes the experience of a long-time childbirth educator who attended the birth of her granddaughter Cora, her daughter Robyn’s second hospital birth. She discusses how Robyn’s instincts and confidence helped to overcome institutional issues to provide a good and safe birthing experience.     

An Integrative Review: The Role of the Doula in Breastfeeding Initiation and Duration

The objective of this integrative review was to assess birth and postpartum doulas'' roles in supporting breastfeeding initiation and duration. The electronic databases Cumulative Index to Nursing and Allied Health Literature (CINAHL), Embase, PubMed, and Scopus were searched using the key terms doula and breastfeeding. Fourteen articles met inclusion criteria. Six key themes were identified. Doulas may acquire only modest amounts of lactation-specific education; however, doula care still enhances the breastfeeding care provided by health-care professionals. Doulas offer prenatal and intrapartum support that encourages breastfeeding initiation in the hospital, as well as providing breastfeeding support in the community and home settings. This reinforces the unique role of the doula in bolstering maternal–infant health. The effect of doulas on breastfeeding duration is less clear.     

Each One Unique

A young mother shares the stories of her two sons'' births. Her first birth experience was complicated by muscle spasms and changes in her baby''s heart rate during labor and, later, by medical problems with her baby in the days after birth. The mother''s strength and commitment shine through at every twist and turn during labor and the days after her son''s birth. Two years later, in contrast to her first son''s birth, the mother''s second son was born so quickly that labor presented a different set of challenges. The mother''s confidence in birth and in herself are essential facets of the stories of the births of her two sons.     

The Birth of Miles Henry DeVries

In this birth story, a second-time mother relates her experience of birthing her son at home after her daughter was born via cesarean surgery. Support from the International Cesarean Awareness Network, as well as a home birth midwife specializing in vaginal birth after cesarean (VBAC), made the dream of a vaginal birth a reality for this mom. This story highlights the importance of having a supportive care provider and laboring in a safe and comfortable environment when pursuing a VBAC.     

The most amazing 24 hours

In this column, a mother shares the story of her son's birth with her prenatal yoga instructor. The mother was able to work with her labor, was well supported by her husband and doula, and negotiated successfully with the hospital staff to have a safe, healthy birth. She describes how difficult labor is and the ways in which comfort and support strategies help women manage contractions.     

Jack Thomas,Our Third Baby,Enters the World!

Without the confidence and support of the medical team, this mother’s first birth was difficult and unsatisfying. She was able to give birth naturally, in the care of a midwife, with her second baby. With confidence in her ability to give birth and the wisdom that comes with experience, she is able to give birth naturally with her third baby in spite of being induced.     

The Birth of Joseph Gabriel

In this column, a mother shares the story of the birth of her first child. With confidence in the process of birth and in her ability to give birth, and with the support, confidence, and encouragement of her mother and sisters, Anne manages to cope with strong contractions through a busy day. Finally, her husband realizes how fast labor is progressing. Baby Joseph was born less than 2 hours after arrival at the hospital.     

Transitions: The Birth of Flora Arden

In this column, a young mother shares the story of her second home birth. Like the birth of her first baby, her daughter Flora’s birth was supported by the presence of a midwife and doula, the peace and quiet of her own home, and the love of her husband and family. Birth is described as transforming and a powerful transition for women. She describes the births of both her children as being “forever emblazoned on my heart as the sweetest and strongest days of my small life.” Paralleling this story of birth is the story of her mother-in-law as she faces brain cancer and seizes the power of that life transition.     

Health-Care Reform for Childbirth

In this column, the editor of The Journal of Perinatal Education discusses the current health-care crisis and the need for health-care reform to promote, support, and protect natural, safe, and healthy childbirth. The editor also describes the contents of this issue, which offer a broad range of resources, research, and inspiration for childbirth educators in their efforts to promote normal birth.     

Elizabeth Belle’s Birth Story

In this article, Jessica and Samuel Boro share the story of the birth of their daughter, Elizabeth Belle. With the physical and emotional support of her husband and her doula, this mother was able to cope with a long labor and have the natural birth she wanted. Her husband describes how important the doula was for him.     

Continuing Education Module: Persuasion: The Key to Changing Women's Ideas About Birth

Although standard maternity care is not evidence-based and, in many cases, increases risks for mothers and babies, few women make birth decisions that reflect this knowledge. This column discusses persuasion as a way to change women''s ideas about safe, healthy birth. The relationship between persuasion and choice is discussed, and the differences between presenting information and persuading women that natural birth is the safest and healthiest way to give birth are explored.     

Making a difference with evidence

In this column, the editor of The Journal of Perinatal Education describes how, despite the many challenges confronting childbirth educators today in our complex health-care environment, there is still a ray of hope that committed individuals translating evidence into practice can make a difference. The editor also describes the contents of this special issue, which celebrates the 50th anniversary of Lamaze International and offers a broad range of resources, research, and inspiration for childbirth educators in their efforts to promote natural, safe, and healthy birth.     

Advisee‐advisor communication: An exploratory study examining interpersonal communication variables in the graduate advisee‐advisor relationship

The objective of this study was to investigate graduate students' perceptions of their graduate advisors' communication (competence, credibility, and nonverbal immediacy), and how these perceptions impact advisees' perceptions of learning, effectiveness of the advisee‐advisor relationship, and advisors' degrees of mentoring. Advisee perceptions of her or his advisor's competence and caring/goodwill accounted for 43% of the variance in advisee cognitive learning. Advisee perceptions of her or his advisor's caring/ goodwill accounted for 39% of the variance in advisee perceptions of the effectiveness of the advisee‐advisor relationship. Lastly, this study noted that the linear combination of advisee perceptions of advisor credibility and communication competence accounted for 55% of the variance in an advisee's perception of the amount of mentoring an advisee received from her or his graduate advisor.     

New Resources for Childbirth Educators and Parents

In this column, reviewers offer perspectives and comments on a variety of new media resources for childbirth educators and for expectant and new parents. The books, CD, and DVDs reviewed in this issue''s column address the following topics: natural, safe, and healthy birth practices; doula care; breastfeeding; empowering women to make healthy lifestyle choices during pregnancy; encouraging mothers to bond with their babies before birth; and fathers'' and partners'' supportive role during labor and birth.     

Reflections on the “Push for Your Baby” Campaign

In this editorial, a board member of Lamaze International describes the “Push for Your Baby” campaign to urge women to advocate for more evidence-based practice for better births. She also reflects on her hopes and worries about the “Push for Your Baby” campaign launched by Lamaze in May 2012. Discussing the realities of current maternity care practice, she asks how we can work with obstetric nurses and providers to have them support what most women value—vaginal birth.
