AbstractThis prospective cohort study described return-to-play (RTP) data for different types of muscle injuries in male elite-level football players in Europe. Eighty-nine European professional teams were followed between 2001 and 2013. Team medical staff recorded individual player exposure and time-loss injuries. A total of 17,371 injuries occurred, including 5603 (32%) muscle injuries. From 2007, we received results from 386 magnetic resonance imaging (MRI) examinations, and radiological grading was performed. A negative MRI was associated with shorter recovery time (6 ± 7 days). Lay-off days were correlated with MRI grading of thigh muscle injuries ( P < 0.001). Among hamstring injuries, 83% occurred to the biceps femoris, 12% affected the semimembranosus and 5% the semitendinosus. Recurrence rate was higher among biceps femoris injuries (18%) compared with semitendinosus and semimembranosus injuries (2% together). Groin muscle injuries caused shorter median absence (9 days) than hamstring (13 days; P < 0.001), quadriceps (12 days; P < 0.001) and calf muscle (13 days; P < 0.001) injuries. Overall, we found that MRI was valuable for prognosticating RTP, with radiological grading associated with lay-off times after injury. Re-injuries were common in biceps femoris injuries but rare in semitendinosus and semimembranosus injuries. 相似文献

Abstract We tested hypotheses grounded in self-determination theory regarding athletes' cognitive appraisals and emotional reactions when returning to competition following a serious injury. Professional male athletes ( N = 225) competing in the Australian Football League were presented with return-to-competition scenarios that varied with respect to: (1) degree of self-determination and (2) salience of re-injury concern. Significant MANOVA main effects were observed for the degree of self-determination on both primary appraisals and emotional responses as well as for the salience of re-injury concern on emotional response. Follow-up ANOVA polynomial trend analyses provided support for the hypothesized trends for greater self-determination in the return to sport to result in more positive appraisals and affect. Findings support self-determination theory contentions and research indicating the psychological benefits of increased self-determination. 相似文献

目的:了解人体前交叉韧带(anterior cruciate ligament,ACL)的损伤机制,并对其损伤治疗与康复手段进行研究,为日后更好的康复手段的探究奠定基础。方法:通过在中国知网与Pub Med数据库中查阅大量中外文献资料进行综述。结果与结论:ACL损伤多是膝关节强力过伸或强力外展造成的。ACL损伤后,膝关节将发生异常活动,易引发ODonoghue三联症。有80%的膝关节韧带手术涉及ACL手术,且手术后还会产生严重的后遗症。随着人们对ACL损伤关注度的增高,手术治疗后伴随的康复治疗手段也日新月异,并证实使用新型康复手段的实验组的康复效果显著高于常规康复组。 相似文献

ABSTRACT The purpose was to quantify the effects of mid-flight whole-body and trunk rotation on knee mechanics in a double-leg landing. Eighteen male and 20 female participants completed a jump-landing-jump task in five conditions: no rotation, testing leg ipsilateral or contralateral (WBRC) to the whole-body rotation direction, and testing leg ipsilateral (TRI) or contralateral to the trunk rotation direction. The WBRC and TRI conditions demonstrated decreased knee flexion and increased knee abduction angles at initial contact (2.6 > Cohen’s dz > 0.3) and increased peak vertical ground reaction forces and knee adduction moments during the 100 ms after landing (1.7 > Cohen’s dz > 0.3). The TRI condition also showed the greatest knee internal rotation angles at initial contact and peak knee abduction and internal rotation angles and peak knee extension moments during the 100 ms after landing (2.0 > Cohen’s dz > 0.5). Whole-body rotation increased contralateral knee loading because of its primary role in decelerating medial-lateral velocities. Trunk rotation resulted in the greatest knee loading for the ipsilateral knee due to weight shifting and mechanical coupling between the trunk and lower extremities. These findings may help understand altered trunk motion in anterior cruciate ligament injuries. 相似文献

The potential to use the vertical jump (VJ) to assess both athletic performance and risk of anterior cruciate ligament (ACL) injury could have widespread clinical implications since VJ is broadly used in high school, university, and professional sport settings. Although drop jump (DJ) and VJ observationally exhibit similar lower extremity mechanics, the extent to which VJ can also be used as screening tool for ACL injury risk has not been assessed. This study evaluated whether individuals exhibit similar knee joint frontal plane kinematic and kinetic patterns when performing VJs compared with DJs. Twenty-eight female collegiate athletes performed DJs and VJs. Paired t-tests indicated that peak knee valgus angles did not differ significantly between tasks ( p = 0.419); however, peak knee internal adductor moments were significantly larger during the DJ vs. VJ ( p < 0.001). Pearson correlations between the DJ and VJ revealed strong correlations for knee valgus angles ( r = 0.93, p < 0.001) and for internal knee adductor moments ( r = 0.82, p < 0.001). Our results provide grounds for investigating whether frontal plane knee mechanics during VJ can predict ACL injuries and thus can be used as an effective tool for the assessment of risk of ACL injury in female athletes. 相似文献

Anterior cruciate ligament (ACL) injury prevention programmes have not been as successful at reducing injury rates in women’s basketball as in soccer. This randomised controlled trial (ClinicalTrials.gov #NCT02530333) compared biomechanical adaptations in basketball and soccer players during jump-landing activities after an ACL injury prevention programme. Eighty-seven athletes were cluster randomised into intervention (6-week programme) and control groups. Three-dimensional biomechanical analyses of drop vertical jump (DVJ), double- (SAG-DL) and single-leg (SAG-SL) sagittal, and double- (FRONT-DL) and single-leg (FRONT-SL) frontal plane jump landing tasks were tested before and after the intervention. Peak angles, excursions, and joint moments were analysed using two-way MANCOVAs of post-test scores while controlling for pre-test scores. During SAG-SL the basketball intervention group exhibited increased peak knee abduction angles ( p = .004) and excursions ( p = .003) compared to the basketball control group ( p = .01) and soccer intervention group ( p = .01). During FRONT-SL, the basketball intervention group exhibited greater knee flexion excursion after training than the control group ( p = .01), but not the soccer intervention group ( p = .11). Although women’s soccer players exhibit greater improvements in knee abduction kinematics than basketball players, these athletes largely exhibit similar biomechanical adaptations to ACL injury prevention programmes. 相似文献

The number and type of landings performed after blocking during volleyball matches has been related to the potential risk of ACL injury. The aim of the present study was to determine whether gender affects the frequency of specific blocking landing techniques with potential risk of ACL injury from the perspective of foot contact and subsequent movement after the block used by volleyball players during competitive matches. Three matches involving four female volleyball teams (fourteen sets) and three matches involving four male volleyball teams (thirteen sets) in the Czech Republic were analyzed for this study. A Pearson chi-square test of independence was used to detect the relationship between gender and different blocking techniques. The results of the present study showed that gender affected single-leg landings with subsequent movement in lateral direction and double-leg landings. Although the total number of landings was lower for male athletes than for female athletes, a larger portion of male athletes demonstrated single leg landings with a subsequent movement than female athletes. Single leg landings with a subsequent movement have a higher potential risk of ACL injury. 相似文献

In lateral reactive movements, core stability may influence knee and hip joint kinematics and kinetics. Insufficient core stabilisation is discussed as a major risk factor for anterior cruciate ligament (ACL) injuries. Due to the higher probability of ACL injuries in women, this study concentrates on how gender influences trunk, pelvis and leg kinematics during lateral reactive jumps (LRJs). Perturbations were investigated in 12 men and 12 women performing LRJs under three different landing conditions: a movable landing platform was programmed to slide, resist or counteract upon landing. Potential group effects on three-dimensional trunk, pelvic, hip and knee kinematics were analysed for initial contact (IC) and the time of peak pelvic medial tilt (PPT). Regardless of landing conditions, the joint excursions in the entire lower limb joints were gender-specific. Women exhibited higher trunk left axial rotation at PPT (women: 4.0 ± 7.5°, men: ?3.1 ± 8.2°; p = 0.011) and higher hip external rotation at both IC and PPT ( p < 0.01). But women demonstrated higher knee abduction compared to men. Men demonstrated more medial pelvic tilt at IC and especially PPT (men: –5.8 ± 4.9°, women: 0.3 ± 6.3°; p = 0.015). Strategies for maintaining trunk, pelvis and lower limb alignment during lateral reactive movements were gender-specific; the trunk and hip rotations displayed by the women were associated with the higher knee abduction amplitudes and therefore might reflect a movement strategy which is associated with higher injury risk. However, training interventions are needed to fully understand how gender-specific core stability strategies are related to performance and knee injury. 相似文献

Anterior cruciate ligament (ACL) injuries commonly occur during jump-landing tasks when individuals’ attention is simultaneously allocated to other objects and tasks. The purpose of the current study was to investigate the effect of allocation of attention imposed by a secondary cognitive task on landing mechanics and jump performance. Thirty-eight recreational athletes performed a jump-landing task in three conditions: no counting, counting backward by 1 s from a randomly given number, and counting backward by 7 s from a randomly given number. Three-dimensional kinematics and ground reaction forces were collected and analysed. Participants demonstrated decreased knee flexion angles at initial contact ( p = 0.001) for the counting by 1 s condition compared with the no counting condition. Participants also showed increased peak posterior and vertical ground reaction forces during the first 100 ms of landing ( p ≤ 0.023) and decreased jump height ( p < 0.001) for the counting by 1 s and counting by 7 s conditions compared with the no counting condition. Imposition of a simultaneous cognitive challenge resulted in landing mechanics associated with increased ACL loading and decreased jump performance. ACL injury risk screening protocols and injury prevention programmes may incorporate cognitive tasks into jump-landing tasks to better simulate sports environments. 相似文献

AbstractIn team sports, non-contact ACL and MCL injuries occur during abrupt changes of direction, like turns or cutting manoeuvres. Fatigue affects dynamic neuromuscular control and increases knee injury risk. This study analysed how lower limb joints and centre-of-mass kinematics are affected throughout a high-intensity running protocol involving repeated 180°-turns. Twenty young men (18–23 years, BMI: 20.8–24.4?kg?m ?2) completed a 5-m shuttle running trial lasting 5?min at an average speed of 75% of their maximum aerobic speed. During the test, cardio-metabolic parameters were obtained, together with joints and centre-of-mass kinematics, using a motion capture system. Kinematic data were compared between the first and the last minute of exercise. Perceived exercise intensity ranged from “hard” to “maximum exertion” and post-exercise lactate concentration ranged from 5.4 to 15.5?mM. The repetition of 180°-turns induced a substantial reduction of hip (?60%, p?<?.001, large effect) and knee flexion (?40%, p?=?.003, medium-to-large effect), and an increase of hip adduction and internal rotation (+25–30%, p?<?.05, medium-to-large effect). Since such movements are factors increasing the likelihood of non-contact knee injuries, we concluded that the prolonged repetition of turns may expose participants to increased risk of ligament failure. Increasing knee stability via appropriate muscle activation could reduce anterior cruciate ligament (ACL) injury risk during unplanned sidestepping. High-level athletes may activate their knee muscles differently from low-level athletes when responding to quasi-game realistic versus non game-realistic stimuli. Eleven high-level and 10 low-level soccer players responded to a non game-realistic arrow-planned condition (AP), a quasi game-realistic one-defender scenario (1DS) and two-defender scenario (2DS), and an arrow-unplanned condition (AUNP), that imposed increasing time constraints to sidestep. Activation from eight knee muscles during sidestepping was measured during pre-contact and weight-acceptance. Knee flexor-extensor co-activation ratios were established. Muscle activation levels increased by approximately 27% solely in the 1DS in both sidestepping phases. In the 2DS, the shift from a flexor dominant co-activation strategy in pre-contact toward extensor dominance in weight-acceptance commenced earlier for the high-level players. Quasi game-realistic information allowed for anticipatory increases in knee muscle activation regardless of expertise levels but only when the time demands to respond were low (1DS). High-level players were better at interpreting complex game-realistic information (2DS) to activate their knee extensors earlier in preparation for single-leg landing during weight-acceptance. 相似文献

The ACL-Return to Sport after Injury scale (ACL-RSI) measures athletes’ emotions, confidence in performance, and risk appraisal in relation to return to sport after ACL reconstruction. Aim of this study was to study the validity and reliability of the Dutch version of the ACL-RSI (ACL-RSI (NL)). Total 150 patients, who were 3–16 months postoperative, completed the ACL-RSI(NL) and 5 other questionnaires regarding psychological readiness to return to sports, knee-specific physical functioning, kinesiophobia, and health-specific locus of control. Construct validity of the ACL-RSI(NL) was determined with factor analysis and by exploring 10 hypotheses regarding correlations between ACL-RSI(NL) and the other questionnaires. For test–retest reliability, 107 patients (5–16 months postoperative) completed the ACL-RSI(NL) again 2 weeks after the first administration. Cronbach’s alpha, Intraclass Correlation Coefficient (ICC), SEM, and SDC, were calculated. Bland–Altman analysis was conducted to assess bias between test and retest. Nine hypotheses (90%) were confirmed, indicating good construct validity. The ACL-RSI(NL) showed good internal consistency (Cronbach’s alpha 0.94) and test–retest reliability (ICC 0.93). SEM was 5.5 and SDC was 15. A significant bias of 3.2 points between test and retest was found. Therefore, the ACL-RSI(NL) can be used to investigate psychological factors relevant to returning to sport after ACL reconstruction. 相似文献

The Injury Psychological Readiness to Return to Sport (I-PRRS) scale measures the psychological readiness of injured athletes to resume sports participation. The aim of this study was to translate and culturally adapt the I-PRRS scale into Dutch (I-PRRS-NL) and assess its validity, reliability, and stability in patients after anterior cruciate ligament reconstruction (ACLR). The original I-PRRS was translated and culturally adapted from English into Dutch, and tested for clinimetric quality. To assess concurrent validity, 150 patients completed the I-PRRS-NL scale and five questionnaires measuring related constructs 3–16 months after ACLR. All predefined hypotheses regarding correlations between the I-PRRS-NL scale and these questionnaires were confirmed, indicating good concurrent validity. For test-retest reliability, 107 patients completed the I-PRRS-NL scale again two weeks later. The I-PRRS-NL scale showed good internal consistency (Cronbach’s alpha 0.94) and test-retest reliability (ICC 0.89). Standard error of measurement was 4.2 and smallest detectable change was 11.6. No systematic bias between test and retest was found. As the scale appears unstable at item level, only the total score should be used. Overall, the I-PRRS-NL scale showed sufficient validity and reliability to assess the psychological readiness to resume sports among Dutch-speaking patients after ACLR. 相似文献

To evaluate the value of ultrasonography in the diagnosis of anterior cruciate ligament injury (ACL injury) by conducting a systematic review and meta-analysis. A literature search was carried out in the Cochrane Library, Embase, Pubmed databases and included studies prior to April 2017. Based on inclusion and exclusion criteria, studies evaluating ultrasound to diagnose ACL injury were selected. MRI, arthroscopy and clinical-follow were considered the reference standards. The diagnostic accuracy of ultrasound was assessed using a combination of sensitivity, specificity, likelihood ratio (LR), post-test probability, diagnostic odds ratio (DOR) and by summarizing the area under the receiver operating characteristic (SROC) curve. A total of 4 studies involving 246 patients were eventually included in the analysis. In these four studies, the combined sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, post-test probability and DOR were 90.0% (95% CI: 77–96), 97% (95% CI: 90–99), 31.08 (95% CI: 8.75–110.41), 0.11 (95% CI: 0.05–0.24), 89% (3%) and 288.81 (95% CI: 78.51–1062.48), respectively. The area under the SROC curve was 0.98 (95% CI: 0.97–0.99). Our meta-analysis showed that ultrasound can play an important role in the diagnosis of ACL injury. Because of its high sensitivity, high specificity and high diagnostic ability, ultrasound should be a part of the standard diagnostic work-up of an ACL injury. 相似文献

This study involved a systematic video analysis of 16 anterior cruciate ligament (ACL) injuries sustained by elite-level netball players during televised games in order to describe the game situation, the movement patterns involved, the player’s behaviour, and a potential injury mechanism. Eight of the ACL injuries were classified as “indirect contact” and eight as “non-contact”. Two common scenarios were identified. In Scenario A the player was jumping to receive or intercept a pass and whilst competing for the ball experienced a perturbation in the air. As a result the player’s landing was unbalanced with loading occurring predominantly on the knee of the injured side. In Scenario B the player was generally in a good position at ground contact, but then noticeably altered the alignment of the trunk before the landing was completed. This involved rotating and laterally flexing the trunk without altering the alignment of the feet. Apparent knee valgus collapse on the knee of the injured side was observed in 3/6 Scenario A cases and 5/6 Scenario B cases. Players may benefit from landing training programmes that incorporate tasks that use a ball and include decision-making components or require players to learn to cope with being unbalanced. 相似文献
