Abstract The aim of this study was to examine the effectiveness of either a standard care programme ( n?=?9) or a 12-week supported exercise programme ( n?=?10) on glycaemic control, β-cell responsiveness, insulin resistance, and lipid profiles in newly diagnosed Type 2 diabetes patients. The standard care programme consisted of advice to exercise at moderate to high intensity for 30?min five times a week; the supported exercise programme consisted of three 60-min supported plus two unsupported exercise sessions per week. Between-group analyses demonstrated a difference for changes in low-density lipoprotein cholesterol only (standard care programme 0.01 mmol?·?L ?1, supported exercise programme –0.6 mmol?·?L ?1; P?=?0.04). Following the standard care programme, within-group analyses demonstrated a significant reduction in waist circumference, whereas following the supported exercise programme there were reductions in glycosylated haemoglobin (6.4 vs. 6.0%; P?=?0.007), waist circumference (101.4 vs. 97.2?cm; P?=?0.021), body mass (91.7 vs. 87.9?kg; P?=?0.007), body mass index (30.0 vs. 28.7?kg?·?m ?2; P?=?0.006), total cholesterol (5.3 vs. 4.6 mmol?·?L ?1; P?=?0.046), low-density lipoprotein cholesterol (3.2 vs. 2.6 mmol?·?L ?1; P?=?0.028), fasting β-cell responsiveness (11.5?×?10 ?9 vs. 7.0?×?10 ?9 pmol?·?kg ?1?·?min ?1; P?=?0.009), and insulin resistance (3.0 vs. 2.1; P?=?0.049). The supported exercise programme improved glycaemic control through enhanced β-cell function associated with decreased insulin resistance and improved lipid profile. This research highlights the need for research into unsupported and supported exercise programmes to establish more comprehensive lifestyle advice for Type 2 diabetes patients. 相似文献

目的:探讨体力活动(PA)不足和久坐行为(SB)与糖代谢异常检出率的关系。方法:对30~69岁具有至少3项DM危险因素者进行糖耐量筛查,采用国际体力活动问卷(IPAQ-SF)调查PA水平和SB,并利用加速度计对问卷进行效度检验。结果:研究纳入195人,平均年龄(51.92±10.64)岁。糖代谢异常检出率为67.18%,检出率随DM危险因素数量的增多而升高。IPAQ-SF中MPA、MVPA以及SB时间与加速度计测量结果的相关系数r分别为0.35、0.37和0.43。受试者中75.9%属于PA不足,同时具有\"PA不足+(年龄≥ 40岁)+超重/肥胖\"三项的受试者最多(48.21%)。在调整了混杂因素之后,与SB ≤ 6h/d组相比,SB>6h/d组受试者的糖耐量异常检出率更高(OR=2.37,95% CI:1.12~5.02)。结论:多种DM危险因素聚集者中糖代谢异常检出率较高,PA不足者较多,SB增加糖代谢异常风险,IPAQ-SF适用于对该人群进行PA评价。建议对糖尿病多危险因素聚集者,尤其是PA不足、SB较多、超重/肥胖者及时筛查并重点关注,降低糖代谢异常发病率。 相似文献

This study compared the energy expenditure (EE) levels during object projection skill performance (OPSP) as assessed by indirect calorimetry and accelerometry. Thirty-four adults (female n = 18) aged 18–30 (23.5 ± 2.5 years) performed three, 9-min sessions of kicking, over-arm throwing, and striking performed at 6-, 12-, and 30-sec intervals. EE was estimated (METS) using indirect calorimetry (COSMED K4b2) and hip-worn accelerometry (ActiGraph GT3X+). EE using indirect calorimetry demonstrated moderate-intensity physical activity (3.4 ± 0.7 METS––30-sec interval, 5.8 ± 1.2 METS––12-sec interval) to vigorous intensity physical activity (8.3 ± 1.7 METS––6-sec interval). However, accelerometry predicted EE suggested only light-intensity physical activity (1.7 ± 0.2 METS––30-sec interval, 2.2 ± 0.4 METS––12-sec interval, 2.7 ± 0.6 METS––6-sec interval). Hip-worn, ActiGraph GT3X+ accelerometers do not adequately capture physical activity intensity levels during OPSP, regardless of differences in skill performance intervals. 相似文献

Background:Participation in physical activity supports greater cardiorespiratory fitness(CRF),a correlate of cognitive control.However,the relationship between muscular fitness(MF)and cognitive control is less clear.The present study investigated the differential relationship of CRF and MF with cognitive control in older adolescents.Methods:This cross-sectional study involved students(1517 years old,n=541,43%female)from 20 secondary schools who completed tests of inhibition(modified flanker task),working memory(n-back task),CRF(Progressive Aerobic Cardiovascular Endurance Run),and MF(standing long jump and push-up test).Multilevel analyses tested the association between CRF or MF and cognitive outcomes while accounting for the influence of the other fitness variable and relevant demographic factors.Results:CRF predicted response accuracy during incongruent flanker trials,the condition requiring greater inhibition.For the working memory task,CRF predicted greater target accuracy and greater d’scores on the 1-back task,requiring lesser amounts of working memory.In the 2-back task,which requires greater amounts of working memory,CRF also predicted greater target and non-target accuracy and d’scores.Comparatively,MF did not predict any cognitive outcomes after adjustment for CRF.Conclusion:CRF was selectively related to better performance during task conditions that require greater amounts of inhibition and working memory.This finding suggests that CRF,but not MF,may benefit cognitive control in older adolescents.This selective influence of CRF on older adolescents’cognition highlights the value of aerobic physical activity. 相似文献

The role of schools and physical education in promoting health, producing a ‘healthy nation’ and in tackling obesity has been increasingly recognised in recent years. In England this is evidenced by various policies, strategies and responses from government that have highlighted schools to be instrumental in addressing health broadly and obesity specifically. In addition, individual schools and teachers at the local level appear to be responding to the obesity issue in varied and different ways. However, we and other researchers feel that the discourse surrounding obesity and some of the reports, messages, policies and measures being taken to tackle it are misleading, misguided and could do more harm than good. At the same time, as physical educators committed to the health and development of young people, we feel unable to ignore the issue and compelled to act. We contend that ‘every child of every size matters’ and can benefit from regular engagement in physical education and physical activity and furthermore that, as a profession, we have a responsibility to provide all young people, of all sizes, with meaningful, relevant and positive physical education and physical activity experiences. Given this, we consider what role physical education can and should realistically, sensibly and safely play in addressing childhood overweight and obesity. Firstly, some of the key ‘facts’, issues and debates concerning obesity are explored and the way in which the issue appears to be being addressed in many schools is critiqued. We then briefly summarise some of the formal guidance and recommendations available to schools on obesity, before concluding with some practical recommendations to support physical education teachers in effectively engaging all children in physical activity both within and beyond physical education. 相似文献

Abstract The aim of this study was to compare in-school and out-of-school physical activity within a representative sample. Socio-demographic, physical activity, and anthropometric data were collected from a random sample of children (250 boys, 253 girls) aged 3–16 years attending nine primary and two secondary schools. Actigraph GT1M accelerometers, worn for seven days, were used to estimate physical activity levels for in-school (typically 09.00–15.00 h), out-of-school (weekday), and weekend periods. Physical activity as accelerometer counts per minute were lower in school versus out of school overall (in school: 437.2 ± 172.9; out of school: 575.5 ± 202.8; P < 0.001), especially in secondary school pupils (secondary: 321.6 ± 127.5; primary: 579.2 ± 216.3; P < 0.001). Minutes of moderate-to-vigorous physical activity accumulated in school accounted for 29.4 ± 9.8% of total weekly moderate-to-vigorous physical activity overall but varied by sector (preschool: 37.4 ± 6.2%; primary: 33.6 ± 8.1%; secondary: 23.0 ± 9.3%; F = 114.3, P < 0.001). Approximately half of the children with the lowest in-school activity compensated out of school during the week (47.4%) and about one-third at the weekend (30.0%). Overall, physical activity during the school day appears to be lower than that out of school, especially in secondary school children, who accumulate a lower proportion of their total weekly moderate-to-vigorous physical activity at school than younger children. As low in-school activity was compensated for beyond the school setting by less than half of children, promoting physical activity within the school day is important, especially in secondary schools. 相似文献

Tools for measuring walking time make use of objective and subjective methods. One subjective approach is to administer physical activity questionnaires (PAQ), mainly because they are inexpensive and easy to give to large groups. The European Prospective Investigation into Cancer and Nutrition (EPIC) study has a brief PAQ (EPIC-PAQ) and includes one question referencing walking time. The objective of this study was to assess the validity of the question about time spent walking included in the EPIC-PAQ. The sample included 200 older adults (113 women). To assess daily walking time, participants responded to the EPIC-PAQ in an interview and wore a portable gait analysis system and physical activity monitor for 48 consecutive hours in free-living condition. Results indicated that the mean of bias between the EPIC-PAQ and objetive measurement was ?64.6 min/day. Also, the correlation was low compared to an objective measurement (rho = 0.196) and was positively correlated with the time spent at speeds below 2.5 mph but the correlation was low (slow walking rho = 0.154 and pace walking rho = 0.163). The EPIC-PAQ shows low correlations with the objective measurement of walking time, that suggests it may be inaccurate and affecting the estimate of the EPIC-PAQ’s PA energy expenditure in this age group. 相似文献

This study investigated the effect of a 12-week pedometer-based intervention on daily step counts of 147 older adults randomly assigned to an intervention or wait-list control group (M age = 72.9 years, SD = 8.8). The intervention group significantly increased their daily step counts after 12 weeks (M = 639, SD = 2,239) and continued to significantly increase during a 12-week maintenance period (M = 680, SD = 1,721). The control group exhibited no change during the control period (M =-393, SD = 2,050) but had a significant increase in daily step counts (M = 1,580, SD = 2,305) when enrolled in the intervention. The pedometer-based intervention was effective in increasing participants' daily step counts. 相似文献

BackgroundThis study systematically synthesized and quantified the relationship linking state laws governing school physical education (PE) to PE attendance and physical activity (PA) in class and throughout the day and week among students in the USA. MethodsA keyword search was performed in PubMed, Web of Science, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Academic Search Complete, and EconLit. Meta-analyses were performed to estimate the effects of state PE laws. ResultsA total of 17 studies were included in the review, and five contributed to the meta-analyses. A total of 8 studies used nationally representative school- or student-level data, three focused on multiple states, and the remaining six examined the PE laws of a single state. The presence and strength of state PE laws were positively associated with PE attendance and the frequency and duration of PA during PE classes and throughout the school day. Compared to those residing in states with weak or no PE laws, students in states with strong PE laws had an additional 0.2 days (95% confidence interval (95%CI): 0.1–0.4) of PE attendance per week and spent an additional 33.9 min (95%CI: 22.7–45.0) participating PE classes per week. State PE laws affected girls’ PA more than boys’. Different aspects of state PE laws tended to affect students’ PE attendance differently. Disparities in the implementation of state PE laws existed across schools. ConclusionFuture studies should adopt objective measures on PE and PA participation and examine the roles schools and districts play in mediating the effect of state PE laws on students’ PE attendance and PA. 相似文献

This paper explores socio-ecological factors and their interplay that emerge from a qualitative study and influence adolescents' physical activity and sport participation. A total of 13 boys and 7 girls active and inactive adolescents, from years 12 and 13 and different types of school (state and private), participated in semi-structured interviews. It followed a purposeful sampling selection from participants who participated in a previous larger study on physical activity levels. Findings highlight the influence of personal and social interplay, such as perceived competence, obesity and teasing from peers, and new social demands and personal preferences in adolescents' (dis)engagement in physical activity and sport. They also stand out how social and environmental factors affect their participation, such as family, boy/girlfriends, physical education and rural/urban places of residence. These factors appear to have an important impact on the construction of (in)active identities during late adolescence. Policy makers, exercise professionals and educators should be aware of these factors in order to improve health promotion strategies and social policies. This study was designed to test the validity of a transtheoretical model's physical activity (PA) stage measure with intention and different intensities of behavior in a large population-based sample of adults living with diabetes (Type 1 diabetes, n?=?697; Type 2 diabetes, n?=?1,614) and examine different age groups. The overall specificity (classified correctly as inactive) for Type 1 diabetes was 69.3% based on the percentage of individuals in precontemplation, contemplation, and preparation not meeting Canada's PA guidelines. Sensitivity (classified correctly as active) was 82.5% based on the proportion of active participants in action and maintenance. In the Type 2 diabetes group, the overall specificity and sensitivity was 63.9% and 88.2%, respectively. No significant differences were found between the diabetes groups for stage distribution patterns, and sensitivity and specificity values. The majority of the study hypotheses related to intention and behaviors were confirmed, providing further supportive evidence for the utility of this PA staging measure for the diabetes population. 相似文献

AbstractThis study examines the effects of parental attitudes on children's physical activity. A measure of parental attitude (their sport participation when being young) that is independent of the child's behaviour is needed. Using data from the sixth wave of the Taking Part Survey ( n =14,002 adults and n =1116 children), a matching estimator analysis is conducted. The results reveal that there are substantial intergenerational transfers of behaviours, in particular for male adults and male children. Furthermore, while an effect is observed for female adults and male children, female children's sport participation is only marginally influenced by female adult's physical activity when growing up. The results imply that promoting sport to the younger generation seems to be a fruitful strategy in creating a sporting habit and promoting sport participation when being older. However, there are challenges to be overcome in promoting female participation. 相似文献

Abstract In the present study, we examined the utility of volitional and forced intentions in predicting participation in physical activities and effort within the theory of planned behaviour. Four hundred and forty-four participants (184 males, 260 females) aged 19.1 ± 3.3 years (mean ± s) completed self-report measures of intentions, attitudes, subjective norms, perceived behavioural control, volitional intentions, forced intentions, and past behaviour in a physical activity context. Six weeks later, they completed self-report measures of physical activity behaviour and effort. Results indicated that volitional intentions and forced intentions contributed to the prediction of effort over and above intentions, attitudes, subjective norms, perceptions of control, past behaviour, and the product terms of attitudes×intentions and subjective norms×intentions. Volitional intentions and forced intentions did not predict participation in physical activities over and above effort. We concluded that volitional intentions and forced intentions assist in the explanation of effort in the context of physical activity behaviour. 相似文献

Abstract A poor physical condition – expressed as physical inactivity and poor physical fitness – is associated with the development of chronic diseases and premature death. Our aim was to evaluate the methods currently available for measuring physical activity and physical fitness in the general population. Physical activity is determined by duration, frequency, and intensity and derives from many different domains, making it difficult to assess over long periods and no feasible general criterion measure exists. Both objective and subjective methods are available. Of the objective methods, accelerometry is the most attractive technology, and is well enough developed for general use in large populations. The advantage of accelerometry is that it is not dependent on the memory of the individual, but its main disadvantage is that it grossly underestimates energy expenditure, due to the lack of registration of certain activities. This may be overcome to a certain extent by combining accelerometry with heart rate monitoring, although this still does not measure activity in different domains. Of the subjective methods, self-report questionnaires are inexpensive and easy to administer. Many questionnaires have been developed, but we require (1) consensus on which measures to use for validation and (2) further development of internationally standardized questionnaires for use in different settings and to address different scientific questions. Many questionnaires correlate well with biological markers and development of chronic diseases, but subjective measurement will always entail a certain degree of misclassification. Furthermore, unstructured physical activity such as housework and gardening may be subject to recall bias. No method appears better to any other, and the choice of instrument will depend on the research question being asked. Future research should combine information from both objective and subjective methods. Physical fitness comprises several components, including cardiorespiratory endurance and muscle strength and endurance. Direct measurement of oxygen consumption is the criterion measure for cardiorespiratory endurance. As regards muscle strength and endurance, only test–retest reliability is available. Hand-held dynamometers greatly facilitate field testing for maximal isometric muscle strength assessment, while force plate measurements can be used for the lower extremities. For endurance, simple tests such as push-ups and sit-ups are reliable. 相似文献

Focus group methodology was used to explore in depth the perceptions of older adults who had participated in a 12-week pedometer-based intervention. Nineteen women and 8 men, ages 55–86 years, volunteered to take part in the focus groups following participation in the intervention. Four focus groups of six to eight participants were scheduled at primary sites in the southern Maine area. Analysis of the data revealed four specific topical areas: (a) factors that led to increases in daily step count, (b) factors that hindered increases in daily step count, (c) benefits of pedometer intervention program, and (d) recommendations on how to improve older adults' physical activity. Overall, focus group participants found this intervention to be beneficial and recommended its expansion to other groups. 相似文献
