How do clinical questions emerge and move toward resolution in the intensive care setting over the course of 24 hours? In a 20-bed surgical intensive care unit in a large, tertiary-care teaching hospital, informationists shadowed clinicians for 2 48-hour periods to record questions, noting when they were asked and whether they were answered. Thirty-eight percent of 112 recorded questions remained unanswered. Some unanswered questions persisted across shifts, and clinicians'' information-seeking behaviors changed over time. Clinical information services can be improved and integrated more fully into clinical workflows based on a fuller understanding of the life cycle of clinical inquiry.  相似文献

This grounded study examined the process of acclimatization or the process of changing perspectives of a person so as to get used to or feel “at home” among a select group of Filipino elderly in a nursing care facility. As institutionalization of the elderly is not typical in the Philippine context, varied responses were analyzed to understand the factors and the course that an elderly person undergoes over a set period of time to achieve acclimatization. Twenty elderly (n = 20) residents were purposively recruited and subjected to an in-depth interview that chronicled their experiences on their previous and present lives and on the process by which they were able to adjust to a relatively new setting. Preliminarily, a robotfoto was devised to obtain demographic data including visitation frequencies and familial relationships, and this was followed by an in-depth interview. Through the constant comparison method, an interesting model called the Hourglass of Acclimatization emerged. This model yielded two distinct phases contributing to successful acclimatization. One is the Conversion phase, or imbibing the main notion of transforming one's perspectives of him or herself and his or her environment; there is also the Immersion phase, which describes how an elderly involves him or herself completely into the life he or she is supposed to live. This emerged model can contribute to the development of nursing interventions focusing on elderly experiences in the entire course of relocation to a new environment other than what they call “home.”  相似文献

The first part of this paper briefly sketches out the idea of subject position by advancing two general types of subject position, showing how they differ and overlap. The second part discusses subject position as it is employed in argumentation, and specifically how it is used as a rhetorical trope to claim or disclaim legitimacy. A diverse range of social structures, for instance teacher teams, professional communities and teacher learning communities, are established to advance collaboration among teachers. In Norway, Interdisciplinary Teacher Teams (ITTs) have become a common way of organising teachers in schools, recommended in a national curriculum reform in 1997. This study explores the internal structure, social meaning and potential resources for learning and development inherent in the planning and coordination of work in ITT meetings. Most studies of teacher teams as well as teacher learning communities are based on teachers’ experiences, expressed in interviews or surveys. The focus of this study is not on what teachers say about teams, but on what teachers say in teams. While most studies have addressed within‐department, subject‐specific teams, this study focuses on interdisciplinary teams. Team‐talk in two ITTs in two different lower secondary schools in Norway has been videotaped and analysed. Four patterns of interaction have been identified – preserving individualism: renegotiating individual autonomy and personal responsibility; coordination: assuring the social organisation of work; cooperation: creating a shared object or enterprise; and sharing: clarifying pedagogical motives. The study illustrates patterns in team‐talk, conceptualises the processes of decision‐making that take place in these ITTs and identifies resources for learning and development inherent in certain forms of interaction. The study contributes to the research literature by both focusing on the details of the interaction in team meetings and analysing the dynamics of the group interaction in the perspective of the situatedness and the object‐orientation of team‐talk.  相似文献

Several theories of grade inflation are discussed in this review article. It is argued that grade inflation results from the substitution of criteria specific to the search for truth by criteria of quality control generated outside of academia. Particular mechanisms of the grade inflation that occurs when a university is transformed into a commercial enterprise, an industrial workshop or an extended family (Alma Mater) are discussed in detail.  相似文献

This is the result of a Survey of diagnostics laboratories in the Asia Pacific (APAC) region with perspectives on India, investigating the three key aspects that are central to the success of a laboratory: quality, cost and speed. This Survey provides a comparison in selected performance indicators in a large number of diagnostic laboratories in a broad range of countries in the APAC region. The Survey provides data on some key performance characteristics such as quality improvement activities, staff productivity and Turnaround Time (TAT). This Survey also demonstrates in India the common issues facing all the laboratories surveyed but also common solutions using a Quality Systems approach which involves Accreditation, customer responsiveness, greater use of IT, automation and Lean principles. Indian laboratories reported less automation and fewer laboratories have Laboratory Information Systems. The productivity by various measures in Indian laboratories was less than in other APAC laboratories. TAT was more commonly monitored in the Indian specimens though there were fewer laboratories compared with the APAC specimens where there were separate TATs for Short Turnaround Time and Routine specimens.  相似文献
