Interrelationship Between Procalcitonin and Organ Failure in Sepsis

Sepsis suffers from lack of specific clinical symptoms which contribute to one of the major causes of mortality. In the present study, our aim was to evaluate the role of a recent biomarker Procalcitonin (PCT) in predicting organ dysfunction. 71 patients admitted with sepsis were included in the study. PCT levels were measured at 0, 24, 72 h and 7th day and sequential organ failure assessment score (SOFA) scores were calculated. PCT levels significantly decreased (p < 0.001) in 89.3 % of surviving patients, whereas, in 60 % non surviving patients the PCT level increased significantly (p < 0.001). A significant positive correlation between PCT and SOFA score was observed in survivors at each hour. These observations indicate that PCT concentration is significantly associated with severity of multi organ dysfunction and also helps in determining the prognosis of septic patients.     

Effect of Pre-Analytical Errors on Quality of Laboratory Medicine at a Neuropsychiatry Institute in North India

Advances in instrument technology and automation have simplified tasks in laboratory diagnostics reducing errors during analysis thereby improving the quality of test results. However studies show that most laboratory errors occur in the pre-analytical phase. In view of the paucity of studies examining pre-analytical errors, we examined a total of 1513 request forms received at our laboratory during a 3 month period. The forms were scrutinized for the presence of specific parameters to assess the pre-analytical errors affecting the laboratory results. No diagnosis was provided on 61.20% of forms. Type of specimen was not mentioned in 61.60% of the forms and 89.25% of all forms were illegible. Critical results were encountered in 17.30% of patients, and of these 76.60% were not communicated due to incomplete forms. Thus, by following standard operating procedures vigorously from patient preparation to sample processing the laboratory results can be significantly improved without any extra cost.     

Stronger Working Memory Reduces Sexual Risk Taking in Adolescents,Even After Controlling for Parental Influences

This study examined the prospective influence of adolescent working memory (WM) on changes in impulsivity and sexual risk taking and assessed whether this relation could be explained by confounding effects of parental influences. Data from 360 community adolescents (M_age = 13.5 ± 0.95 years; 52% female; 56% non‐Hispanic White; low‐mid socioeconomic status (SES); recruited from Philadelphia area in 2004–2005) were analyzed using structural equation modeling to predict changes in impulsivity and sexual risk taking over a 2‐year follow‐up, using baseline assessments of WM, parental monitoring, parental involvement, and SES. Stronger WM predicted reduced involvement in sexual risk taking at follow‐up, effects channeled through changes in impulsivity dimensions of “acting without thinking” and “inability to delay gratification.” Parental variables had a protective influence on adolescent impulsivity and risk involvement, but the effects of WM operated independently of parental influences.

Study of some common biochemical bone turnover markers in postmenopausal women

Markers of bone formation in serum include total and bone specific alkaline phosphatase, osteocalcin and Type 1 collagen carboxy terminal extension peptide. Bone resorption can be assessed by measuring plasma tartarate resistant acid phosphtase and urinary excretion of collagen degradation products: hydroxyproline, hydroxylysine glycosides and more recently the pyridinium crosslinks and associated peptides. We compared the excretion of hydroxyproline in women of reproductive age group to those of menopausal age group and found a significant difference in the two age groups. Urinary hydroxyproline was found to be significantly raised in post menopausal women. Thus hydroxyproline maybe used as the earliest indicator in the prognostic assessment of postmenopausal women of their risk of developing osteoporosis and fracture.     

Polymorphisms of CYP1A1, GSTM1 and GSTT1 Loci as the Genetic Predispositions of Oral Cancers and Other Oral Pathologies: Tobacco and Alcohol as Risk Modifiers

Polycyclic aromatic hydrocarbons of tobacco require activation by phase I enzymes, such as cytochrome-P4501A1 (CYP1A1) to become an ultimate carcinogen, which are subjected to detoxification by phase II enzymes, especially glutathione S-transferases (GSTs). A study was designed to find whether genetic predisposition are risk modifiers of oral pathologies. The study included 102 cases with Oral Cancers (OCs), 68 cases with nonmalignant pathologies, 100 cases as control group. GSTM1 null genotype was associated with increased risk of OCs but not with benign pathologies. Deleted GSTT1 was associated with all pathologies. Both m1m2 and m2m2 polymorphisms of CYP1A1 were associated with oral pathologies.
