Immune dysregulation in COVID-19 is the major causal factor associated with disease progression and mortality. Role of monocyte HLA-DR (mHLA-DR), neutrophil CD64 (nCD64) and Immune dysregulation index (IDI) were studied in COVID-19 patients for assessing severity and outcome. Results were compared with other laboratory parameters. Antibody bound per cell for mHLA-DR, nCD64 and IDI were measured in 100 COVID-19 patients by flow cytometry within 12 h of hospital admission. Thirty healthy controls (HC) were included. Clinical and laboratory parameters like C - reactive protein (CRP), Procalcitonin (PCT), Absolute Lymphocyte count (ALC), Absolute Neutrophil count (ANC) and Neutrophil to Lymphocyte ratio (NLR) were recorded. Patients were followed up until recovery with discharge or death. Parameters from 54 mild (MCOV-19), 46 severe (SCOV-19) and 30 HC were analysed. mHLA-DR revealed significant and graded down regulation in MCOV-19 and SCOV-19 as compared to HC whereas IDI was lowest in HC with increasing values in MCOV-19 and SCOV-19. For diagnostic discrimination of MCOV-19 and SCOV-19, IDI revealed highest AUC (0.99). All three immune parameters revealed significant difference between survivors (n = 78) and non-survivors (n = 22). mHLA-DR < 7010 and IDI > 12 had significant association with mortality. Four best performing parameters to identify patients with SCOV-19 at higher risk of mortality were IDI, NLR, ALC and PCT. mHLA-DR and IDI, in addition to NLR and ALC at admission and during hospital stay can be utilized for patient triaging, monitoring, early intervention, and mortality prediction. IDI reported for the first time in this study, appears most promising. Methylenetetrahydrofolate reductase (MTHFR) is a critical enzyme of folate pathway and required for DNA synthesis and methylation. MTHFE C677T polymorphisms is reported as risk factors for various diseases and disorders like birth defects, metabolic, neurological, psychiatric disorders, and cancers. Several studies have investigated association between the MTHFR C677T polymorphism and male infertility. To assess the risk associated with MTHFR C677T polymorphism in Asian population, a meta-analysis was performed. Included articles were collected from the following electronic databases: PubMed, Google Scholar, and Science direct up to March 2015. Risk was estimated as pooled odds ratios (ORs) with confidence intervals (CIs) for assessment. Seventeen case–control studies involving 4392 breast infertile males and 3667 fertile males were found suitable for the inclusion in the present meta-analysis. Results showed that the C677T polymorphism was significantly associated with male infertility in Asian population using all the five genetic models (ORT vs. C (allele contrast model) = 1.86, 95% CI 1.7–2.0; ORTT vs. CC (homozygote model) = 1.96, 95% CI 1.67–2.30; ORCT vs. CC (co-dominant model) = 1.40, 95% CI 1.18–1.62; ORTT+CT vs. CC (dominant model) = 1.53, 95% CI 1.30–1.77; ORTT vs. CT+CC (recessive model) = 1.67, 95% CI 1.44–1.92). In conclusion, results of present meta-analysis strongly supported an association between C677T polymorphism and male infertility in Asians.  相似文献

The association between methylenetetrahydrofolate reductase (MTHFR) C677T polymorphism and susceptibility to Alzheimers disease (AD) was controversial in previous studies. The present meta-analysis was designed to investigate the association of MTHFR C677T polymorphism with AD. Nine studies were identified by search of PubMed, Google Scholar, Elsevier, Springer Link databases, up to January 2013. Odds ratios (ORs) with corresponding 95 % confidence interval (CI) were calculated using fixed effects model or random effects model. All statistical analysis was done by Mix version 1.7. MTHFR C677T polymorphism had a significant association with susceptibility to AD in all genetic models (for T vs C: OR 1.29, 95 % CI 1.15–1.44, p < 0.0001; for TT + CT vs CC: OR 1.38, 95 % CI 1.16–1.364, p = 0.0002; for TT vs CC: OR 1.60, 95 % CI 1.25–2.04, p = 0.0001; for CT vs CC: OR 1.28, 95 % CI 1.1–1.53, p < 0.008; for TT vs CT + CC: OR 1.37, 95 % CI 1.12–1.67, p = 0.002). Results from present meta-analysis supported that the MTHFR C677T polymorphism was associated with an increased risk of AD in Asian population.  相似文献

ABSTRACT

Online learning as a concept and as a keyword has consistently been a focus of education research for over two decades. In this paper, we present results from a systematic literature review for the definitions of online learning because the concept of online learning, though often defined, has a range of meanings attached to it. Authors and scholars use the term to mean very distinct, if not contradictory concepts. We conducted systematic literature for over the last 30 years (1988 to 2018) to investigate the number and content of definitions of online learning. We collected 46 definitions from 37 resources and conducted a content analysis on these sets of definitions. Content analysis of the collected definitions led to an understanding of the core elements for defining online learning, the confusion surrounding the terms and the synonyms used for online learning. An evolution of the definition of the concept of online learning was also mapped to the evolution of technology in the last three decades.  相似文献

There is a growing national recognition that teachers and teaching are at the heart of successful educational reform. However, few tools exist for measuring classroom instruction. The primary purpose of this article is to describe methods we developed to measure and study teaching, specifically while teachers were using a multimedia intervention for promoting scientific problem solving. Lessons were videotaped, and coding schemes were developed to measure 2 aspects of teaching: (a) the lesson's organization, particularly whole-class instruction used to introduce problems and share students' work; and (b) the nature of tasks and questions given to students. Results showed that the coding schemes were reliable and that they detected differences in instruction across teachers. Qualitative analyses were consistent with the quantitative findings. The codes also captured features of teaching that would have been difficult to detect or verify with qualitative observations alone. Finally, we explored how these measures could be used with student outcome data to examine the relationship between teaching and learning in future studies. We argue that quantitative measures of instruction serve many purposes, not the least of which is allowing researchers to explore the relationship between teaching and student learning at a high degree of granularity.  相似文献

Methylenetetrahydrofolate reductase (MTHFR) is essential for DNA biosynthesis and the epigentic process of DNA methylation. It has been reported that abnormal DNA methylation contributes to the pathogenesis of congenital anomalies. There were many published case control studies assessing the associations of MTHFR C677T polymorphism with risks of nosyndromic cleft lip with and without palate (nsCL/P), but with inconsistent results. To derive a more precise estimation of the relationship, a meta-analysis was performed. Eligible articles were identified by search of databases including PubMed, Science Direct, Google Scholar and Springer Link up to December, 2015. Finally, a total of 22 studies with 3724 nsCL/P cases and 5275 controls were included in the present meta-analysis. Odds ratios (ORs) with corresponding 95% confidence intervals (95% CIs) were pooled to assess the association. Subgroup analysis based on ethnicity was also performed. All statistical analyses were done by MIX program. Meta-analysis results suggested that MTHFR C677T polymorphism contributed to the increased nsCL/P risk in overall population using four genetic models except homozygote model (for T vs. C: OR = 1.24, 95% CI = 1.1–1.4; for TT + CT vs. CC: OR = 1.29, 95% CI = 1.04–1.59; for CT vs. CC: OR = 1.26, 95% CI = 0.98–1.63; for TT vs. CC: OR = 1.02, 95% CI = 0.74–1.4; for TT vs. CT + CC: OR = 1.36, 95% CI = 1.05–1.74). In conclusion, results of present meta-analysis suggested that MTHFR C677T polymorphism is significantly associated with nonsyndromic orofacial cleft.  相似文献

In this paper, history of secondary school science in Mauritius is traced to identifyfactors, which may have influenced its link to technology of local significance. Theeconomy was based on agriculture. Sugar research was not only well developed, itwas crucial for sustaining the social status of the Franco Mauritian economic elite.Secondary school science on the other hand was unaffected by locally conductedscientific and technical research. It prepared students for examinations of Britishexamination boards and for further education in British universities. This resultedin narrowing of the curriculum to the detriment of important but `non-examinable'components of the syllabus. This `teaching to test' was further enhanced by limitededucational opportunities at each level. All attempts at introducing agriculture andpractical work in the curriculum were also futile because of the apathy descendantsof Indian and African immigrants had for agriculture.Today, there are many educational and vocational opportunities still the educationsystem founded on literary lines with examinations and subsequent selection atdifferent levels has become so deep rooted that any deflection from examinationrequirements is difficult.  相似文献
