股部外伤致肠系膜下动脉栓塞1例

目的:分析股部刺伤与肠系膜动脉栓塞的关系,为防止临床上今后发生类似误诊提供参考;方法:结合临床误诊病例,联系基础医学理论进行分析并行开腹探查;结果:开腹探查证实该例股部刺伤患者急性腹痛乃因肠系膜下动脉的分支———乙状结肠动脉栓塞、乙状结肠坏死所致;结论:股部外伤,尤其是严重刺伤导致大血管破裂并脂肪组织严重损伤时可能引起其他器官脂肪栓塞。

寻根文学的叙事文本综合运用了时间顺序的各种变异形态,如倒叙,预叙,插叙和补叙等。另外,寻根文学作品包含一个表层时间和一个深层时间。表层时间被作者淡化或者直接模糊,为的是摆脱现实时间所带来的沉重感。而深层时间却是作家们着力突出和渲染的。在深层时间里,寻根作家们深入挖掘了民族传统文化,重新审视了历史。     

Serum Paraoxonase (PON1) Activity in North-West Indian Punjabi’s with Acute Myocardial Infarction

Human serum paraoxonase-1 (PON1), an enzyme on HDL prevents oxidation of LDL thereby preventing the development of atherosclerosis. Studies done so far have lead to conflicting results. As studies are lacking in North-West Indian Punjabi’s, a distinct ethnic group with high incidence of coronary artery disease, we determined PONase activity in this population. It has been postulated that sudden lowering of serum PONase may lead to precipitation of acute myocardial infarction. We determined serum PONase activity and lipids in 100 patients each of AMI (within 24 h of onset), stable CAD and 100 age and sex matched healthy controls. These were again determined after 6 weeks in AMI patients. The mean serum PONase activity was lowest in AMI patients (23.26 U/ml) followed by stable CAD patients (102.0 U/ml) where as in controls was highest (179.8 U/ml). In patients with AMI, activity was significantly higher at 6 weeks as compared to that after acute event (49.39 %; p < 0.05). Sudden lowering of serum PONase activity in a population which already has lower activity may be one of the risk factors for development of AMI.     

Chicken or the egg: Longitudinal analysis of the causal dilemma between goal orientation,self-regulation and cognitive processing strategies in higher education

The aim of this study was to investigate the direction of the effect between goal orientation, self-regulation and deep processing strategies in order to understand the impact of these three constructs on students’ achievement. The participants were 110 freshmen from the engineering faculty at the Université catholique de Louvain in Belgium, who were followed during the first three years of their university studies. Data were analyzed through structural equation modeling. The main finding was that mastery goal orientation increased students’ subsequent deep processing which in turn enhanced subsequent self-regulation. Deep processing and self-regulation also appeared to be mutually influential from year 2 to year 3. The implications of our results for the understanding of the interplay between cognitive and motivational processes in higher education are discussed.     

Coronary arterial spasm in single right coronary artery

We presented a case of anomalous single-coronary artery detected incidentally during routine coronary angiography. A 32-year-old male Chinese patient presented with recurrent pre-syncope and six episodes of syncope. Coronary angiography and coronary-computed tomography (CT)-angiography performed by a dual-source computed tomography (DSCT) revealed that the patient had a single large right coronary artery. A moderately large branch originated from the proximal part of the single right coronary artery and extended to the left, passing the anterior to the pulmonary artery, and divided into the anterior descending artery branch and circumflex branch at the base of the left auricular appendage. The episodes of the syncope were suspected to be caused by coronary arterial spasm, so this patient was on a regimen of 30 mg of diltiazem every 6 h and had no recurrence of syncope during follow-up.

食管颈段动脉供血体系的解剖学观察

为探讨食管颈段动脉供血体系构筑状况,以期为临床应用补充解剖学资料,利用成、幼尸体25具,红色乳胶灌注后剥离,对其分支吻合进行解剖学观察与系统分析;另取新鲜胎尸25具,其中墨汁灌注20具,钙钴法碱性磷酸酶染色5具后,组织切片,光学显微镜下观测。得出该段食管分支复杂、吻合丰富,各层均存在动脉网,粘膜下层动脉网最为丰富。
