In December 2019, Wuhan city in the Hubei province of China reported for the first time a cluster of patients infected with a novel coronavirus, since then there has been an outburst of this disease across the globe affecting millions of human inhabitants. Severe acute respiratory syndrome coronavirus type-2 (SARS-CoV-2), is a member of beta coronavirus family which upon exposure caused a highly infectious disease called novel coronavirus disease-2019 (COVID-19). COVID-19, a probably bat originated disease was declared by World Health Organization (WHO) as a global pandemic in March 2020. Since then, despite rigorous global containment and quarantine efforts, the disease has affected nearly 56,261,952 laboratory confirmed human population and caused deaths of over 1,349,506 lives worldwide. Virus passes in majority through respiratory droplets and then enters lung epithelial cells by binding to angiotensin converting enzyme 2 (ACE2) receptor and there it undergoes replication and targeting host cells causing severe pathogenesis. Majority of human population exposed to SARS-CoV-2 having fully functional immune system undergo asymptomatic infection while 5–10% are symptomatic and only 1–2% are critically affected and requires ventilation support. Older people or people with co-morbidities are severely affected by COVID-19. These categories of patients also display cytokine storm due to dysfunctional immune response which brutally destroys the affected organs and may lead to death in some. Real time PCR is still considered as standard method of diagnosis along with other serology, radiological and biochemical investigations. Till date, no specific validated medication is available for the treatment of COVID-19 patients. Thus, this review provides detailed knowledge about the different landscapes of disease incidence, etiopathogenesis, involvement of various organs, diagnostic criteria’s and treatment guidelines followed for management of COVID-19 infection since its inception. In conclusion, extensive research to recognize novel pathways and their cross talk to combat this virus in precarious settings is our future positive hope.  相似文献

Objective:In regard to locating clinical trials for a systematic review, limited information is available about how librarians locate clinical trials in biomedical databases, including (1) how much information researchers provide librarians to assist with the development of a comprehensive search strategy, (2) which tools librarians turn to for information about study design methodology, and (3) librarians'' confidence levels in their knowledge of study design methodology. A survey was developed to explore these aspects of how a medical librarian locates clinical trials when facilitating systematic reviews for researchers.Methods:In this cross-sectional study, a 21-question survey was sent to medical librarians via several email listservs during April 2020. Respondents were limited to librarians who make the decisions on search terms for systematic reviews.Results:Responses (n=120) indicated that librarians were often asked to search for various types of clinical trials. However, there was not a consistent method for creating search strategies that locate diverse types of clinical trials. Multiple methods were used for search strategy development, with hedges being the most popular method. In general, these librarians considered themselves to be confident in locating trials. Different resources were used to inform study types, including textbooks, articles, library guides and websites.Discussion:Medical librarians indicated that while they felt confident in their searching skills, they did not have a definitive source of information about the various types of clinical trials, and their responses demonstrated a clear need and desire for more information on study design methodology.  相似文献

Multisite trials, which are being used with increasing frequency in education and evaluation research, provide an exciting opportunity for learning about how the effects of interventions or programs are distributed across sites. In particular, these studies can produce rigorous estimates of a cross-site mean effect of program assignment (intent-to-treat), a cross-site standard deviation of the effects of program assignment, and a difference between the cross-site mean effects of program assignment for two subpopulations of sites. However, to capitalize on this opportunity will require adequately powering future trials to estimate these parameters. To help researchers do so, we present a simple approach for computing the minimum detectable values of these parameters for different sample designs. The article then uses this approach to illustrate for each parameter, the precision trade-off between increasing the number of study sites and increasing site sample size. Findings are presented for multisite trials that randomize individual sample members and for multisite trials that randomize intact groups or clusters of sample members.  相似文献

Abstract

This article provides practical guidance for researchers who are designing studies that randomize groups to measure the impacts of educational interventions. The article (a) provides new empirical information about the values of parameters that influence the precision of impact estimates (intraclass correlations and R 2 values) and includes outcomes other than standardized test scores and data with a three-level structure rather than a two-level structure, and (b) discusses the error (both generalizability and estimation error) that exists in estimates of key design parameters and the implications this error has for design decisions. Data for the paper come primarily from two studies: the Chicago Literacy Initiative: Making Better Early Readers Study (CLIMBERS) and the School Breakfast Pilot Project (SBPP). The analysis sample from CLIMBERS comprised 430 four-year-old children from 47 preschool classrooms in 23 Chicago public schools. The analysis sample from the SBPP study comprised 1,151 third graders from 233 classrooms in 111 schools from 6 school districts. Student achievement data from the Reading First Impact Study is also used to supplement the discussion.  相似文献

Abstract

The purpose of this paper is to document smallholder experiences during a participatory experimental initiative and draw useful lessons for field practitioners. The main methods used to collect data were participant observation, in-depth interviews among 40 farmers, and analyses of notes taken during participatory monitoring and evaluation. These farmers were regular participants in four collective trials—designed by scientists, managed by smallholders—that were run for over five years in western Kenya. This paper shows that scientific trial designs enhanced ‘conviction’ among smallholders in new technologies that were being experimented. The study further reveals that (1) collective trials must last long enough for interactive learning to effectively occur, (2) farmers’ selected aspects of experiments and did not generally treat the whole trial as one concept, (3) field trials were forms of evidence of research or project activity, and (4) trials were seen as like ‘churches’ and multipurpose congregating sites, especially because these collective experiments had roots in local traditions.

The practical implication is that because interactive trials are social events, like other learning platforms, they must be appropriately situated within local contexts to enhance their relevance. This paper also shows that experiments are only a first step in learning, which needs to be supported by in-depth research and support for smallholder experiments. Collaborative experimentation is an effective tool for improving smallholder knowledge. Experimentation is a hands-on tool; it strengthens interactive learning, enhances understanding of a concept and improves ownership of the process or research among farmers. However, longer-term collective experiments develop a strong underlying human dimension. Understanding the social dynamics that influence the value of collective experimentation is critical for the process of scaling out.  相似文献
