目的:探讨终止14~20周妊娠的最佳方法.方法:把孕14~20周要求终止妊娠的健康孕妇随机分为两组,对照组直接羊膜腔内注射利凡诺100 mg,研究组分次口服完200 mg米非司酮后再行羊膜腔内注射.并观察进入产程时间、总产程时间、产后出血量、子宫持续出血时间、胎盘胎膜残留率、清宫率、引产成功率等.结果:研究组比对照组进入产程快,总产程短,产后出血量少,子宫持续出血时间短,胎盘胎膜残留少,清宫率低,差异有显著性.引产成功率差异无显著性.结论:米非司酮配伍利凡诺终止中期妊娠优于单纯使用利凡诺引产,方法简单可行.值得临床推广.  相似文献

Chiropractic and medical colleges have experienced a significant increase in the number of female applicants in recent years, a percentage of whom are pregnant or become pregnant following admission. It is therefore important to ask the question: How do institutions that educate future health care providers address the issue of pregnancy and the gross anatomy laboratory? A survey instrument was developed and pretested. IRB approval was obtained. The administrators charged with overseeing the policies and practices for the gross anatomy laboratory at each of the 16 chiropractic colleges in the USA were identified and contacted. An email containing a link to the Web based survey was sent to each, using SurveyMonkey. The survey response rate was 100%. A majority of colleges (69%) have a written policy regarding pregnancy and the gross laboratory. Of these, 36% allow pregnant students to take the laboratory if a waiver is signed, 18% do not allow them to take the laboratory, 18% allow them to take it without a waiver, and 27% have other policies. In cases where students do not take the gross laboratory while pregnant, 64% of colleges require them to take the laboratory after completion of their pregnancy, 27% require them to complete an alternative (dry) laboratory, and 9% have other policies. Considerable diversity exists in the way colleges address this issue. It is at present unknown whether pregnant students or their fetuses are at any risk from laboratory chemicals. Herpes during pregnancy is a common concern. Because one in every five women is infected with the herpes virus, it is likely that herpes may be a topic that will be raised during a prenatal class, or afterward in private. An expectant mother who has a history of herpes may have concerns about protecting her baby from this virus. She may also have concerns about her own health, relationship issues including the support of her husband and family, or how to communicate with her health-care provider and make the best choices for a safe birth experience. 通过分析影响人工授精的各种因素寻求提高临床妊娠率的方法.方法:对行IUI治疗的不孕症夫妇148例共242个周期,分析患者的年龄、促排卵方案、HCG注射日子宫内膜厚度及类型、每周期内IUI次数、IUI周期数、处理后前向运动精子总数对IUI妊娠率的影响.结果:女方年龄、助孕周期数、HCG注射日子宫内膜厚度及类型、精液处理后的前向运动精子总数与IUI妊娠率有关.结论:女方年龄、助孕周期数、HCG注射日子宫内膜厚度及类型、精液处理后的前向运动精子总数影响IUI妊娠率,一周期内双次IUI、促排卵药物使用无法提高IUI妊娠率.  相似文献

There is a strong likelihood that at least one participant in any Lamaze childbirth education class has had personal experience with childhood sexual abuse. Using the wisdom of Lamaze Certified Childbirth Educators and respected authors in the field of childhood sexual abuse, this column enlightens the childbirth educator in three ways: understanding the incidence of female and male childhood sexual abuse; understanding the effects of sexual, emotional, physical, and verbal abuse on the pregnant and parenting family; and facilitating classes that are sensitive to the needs of survivors of childhood sexual abuse as well to all expectant parents.  相似文献

The aim of this study was to describe women''s experience of pregnancy. The setting was the Alternative Birth Center at Sahlgrenska University Hospital in Göteborg, Sweden, from 1996 to 1997. Anonymous diaries were written by 12 women and analyzed using a hermeneutical/phenomenological approach. Despite an overall reduction in teenage pregnancy rates in the USA, the decrease for young women of Mexican heritage in the USA has been less significant than the decrease for their White and African-American peers. Furthermore, the availability of teenage pregnancy prevention models that are conceptualised specifically for people of Mexican descent is minimal. Using qualitative focus group data that were triangulated against quantitative survey data and collected from 35 young people and their parents, this paper presents findings from the pilot of a teenage pregnancy prevention model designed for and with input from Mexican American young women and men and their parents. Reflecting a favourable and impactful experience for the participants, the findings present implications for future intervention efforts and research.  相似文献

The purpose of this study was to examine women's leisure time physical activity (LTPA) before pregnancy, during pregnancy, and through the first 7 months postnatal. Pre- and postnatal women (n = 309) completed the 12-month Modifiable Activity Questionnaire and demographic information. Multilevel modeling was used to estimate a growth curve representing the average change in LTPA over time and intraindividual variations in the average growth curve over time. Growth curve estimates for the linear, quadratic, and cubic trends were significant (p < .05), indicating that LTPA declined during pregnancy but then increased following birth. The results also demonstrated that the individual trajectories of LTPA varied substantially from the average growth curve. One demographic predictor variable (having other children at home) was significant (p < .05).  相似文献

Abstract

The aims of the this prospective study were to analyse physical activity (PA) engagement during the first and second trimesters, considering the different guidelines published on PA, to document the individual characteristics associated with the accomplishment of these guidelines and to examine pregnant women’s perceived barriers to leisure PA, using a socioecological framework. A sample of 133 pregnant women in two stages – at 10–12 weeks’ gestation (T1) and 20–22 weeks’ gestation (T2) – were evaluated. PA was assessed by accelerometry during the T1 and T2 evaluation stages. Socio-demographic characteristics, lifestyle factors and barriers to leisure PA were assessed via questionnaire. A large proportion of women (ranging from 32% to 96%) did not reach the levels of PA recommended by the guidelines. There were no significant differences between T1 and T2 with regard to compliance with PA recommendations. A decrease in PA levels from T1 to T2 was noted for all recommendations. No associations were found between participants’ characteristics and adherence to the recommendations in T1 and T2. No significant differences were found in barriers to leisure PA between T1 and T2. The most commonly reported barriers to leisure PA were intrapersonal, not health related. Our results indicate that there were no differences between trimesters regarding compliance of PA recommendations, and perceived barriers were similar in both trimesters.  相似文献

This analysis was conducted to describe the concept of therapeutic alliance and its appropriateness for health-care provider-client interactions during the childbearing season. The concept has been defined in other disciplines. A universal definition suggested a merging of efforts directed toward health. A simple and concise definition evolved, which is applicable to the childbearing season as well as to health-care encounters across the life span. This definition states: Therapeutic alliance is a process within a health-care provider-client interaction that is initiated by an identified need for positive client health-care behaviors, whereby both parties work together toward this goal with consideration of the client''s current health status and developmental stage within the life span.  相似文献
