Case Report: CD19-positive acute myeloblastic leukemia with trisomy 21 as a sole acquired karyotypic abnormality

We report that a 63-year-old Chinese female had acute myeloblastic leukemia (AML) in which trisomy 21 (+21) was found as the sole acquired karyotypic abnormality. The blasts were positive for myeloperoxidase, and the immunophenotype was positive for cluster of differentiation 19 (CDI9), CD33, CD34, and human leukocyte antigens (HLA)-DR. The chromosomal analysis of bone marrow showed 47,XX,+21 [2]/46,XX[18]. Fluorescent in situ hybridization (FISH) showed that three copies of AML1 were situated in separate chromosomes, and that t(8;21) was negative. The patient did not have any features of Down syndrome. A diagnosis of CD19-positive AML-M5 was established with trisomy 21 as a sole acquired karyotypic abnormality. The patient did not respond well to chemotherapy and died three months after the diagnosis. This is the first reported case of CD19-positive AM L with trisomy 21 as the sole cytogenetic abnormality. The possible prognostic significance of the finding in AML with +21 as the sole acquired karyotypic abnormality was discussed.

The Rise of the Small: The Effects of Industry Consolidation on Small Legal Publishers

Summary

The legal publishing industry has experienced waves of acquisition and consolidation, with almost all of the established players becoming affiliated with either Thomson or Reed Elsevier. A widespread assumption is that this consolidation is driving out the smaller participants in the market, especially in conjunction with the expansion of free Web sites for primary source material. It is presumed that the entrepreneurs who have smaller, profit-based companies are becoming less interested in the market due to financial pressures, increased difficulty of capturing market share, and threat of acquisition (friendly or hostile).

In order to confirm or refute this assumption, the author reviews the industry consolidations, describes the delineation of representative small legal publishers, and analyzes their responses to questions about how the recent industry changes have affected their strategies, markets and product lines. The resulting picture is of an environment much different than that painted by current media pundits. Contrary to the presumption of a disappearing landscape of small players, there is a positive perception of “ripe opportunity” pushing companies into this marketplace-companies armed with new technology tools and strong convictions that they can reach customers ready and willing to do old business in new ways. The small publishers seem to be weathering the climactic industry changes, and have expectations of growth and expansion.

慢性支气管炎急性发作期危重病人免疫功能的研究

对慢性支气管炎急性发作期和缓解期患者机体免疫功能进行了研究,用荧光单抗ＣＤ^ 3、ＣＤ^ 25对Ｔ淋巴细胞进行标记和计数。检测结果,慢性支气管炎急性发作期患者内Ｔ淋巴细胞活化率降低,造成Ｔ淋巴细胞总数减少,认为因慢性支气管炎急性期患者Ｔ淋巴细胞总数养活和免疫功能紊乱,因此在治疗疾病过程中除对症处理外,还须配合使用提高功能药物,改善免疫功能。
