Genetic Polymorphism of Angiotensin II Type 1 Receptors and Their Effect on the Clinical Outcome of Captopril Treatment in Arab Iraqi Patients with Acute Coronary Syndrome (Mid Euphrates)

Genetic variation in the angiotensin II type 1 receptor (AT1R) has an important effect on the outcome of acute coronary syndrome (ACS) initiated treatment with captopril. This study aims to investigate the impact of genetic polymorphism of AT1R (rs5186 and rs275651) on the ACS outcome in Iraqi patients treated with captopril. A total of 250 Iraqi individuals with ACS were included in this case—control study and they were divided into two study groups; Study group 1 included 125 participants who were prescribed captopril, 25 mg twice daily and study group 2 included 125 participants who received no captopril as part of their ACS treatment (control study). The AT1R gene (rs5186) CC genotype was found to be associated with ST-elevation myocardial infarction (STEMI) (Odd’s ratio (O.R) = 1.2, P = 0.7), while AC was associated with Non-ST-elevation myocardial infarction (NSTEMI) and unstable angina (UA) (O.R = 1.2, P = 0.8). AC genotype is more prone to have Percutaneous coronary intervention (PCI) after ACS attack (O.R = 1.2, P = 0.6). CC genotype had a risk to get less improvement (O.R = 1.6, P = 0.5), so might require higher doses of captopril during acute coronary insult. The AT1R gene (rs275651) AA genotype was associated with UA (O.R = 1.3, P = 0.9). AA and AT genotypes were more prone to have PCI after ACS attack (O.R = 3.9 P = 0.2, O.R = 3.5, P = 0.3 respectively) and thus requiring higher doses of captopril. We conclude that the AT1R rs5186, rs275651 genetic polymorphisms might partially affect the clinical outcome of ACS patients treated with captopril and might have captopril resistance which requires higher doses.     

Interactive Storytelling: From the Book of Genesis

Keith Park, advisory teacher for Sense (the National Deafblind Rubella Association) in Greenwich and Lewisham, London, has written about his approach to interactive storytelling for BJSE before. This article describes a series of poetry workshops based on chapters 37 to 45 of the Book of Genesis (the story of Joseph and his family) using the text of the King James Bible of 1611. The workshops took place at Charlton School in Greenwich between September and November 2002. The full text of the Genesis workshops, with a clear exposition of their implementation, is provided here but the article begins with a restatement of the principles of interactive storytelling. The importance of developing curriculum access for pupils with multi-sensory impairments while simultaneously supporting staff who work with these young people in schools is emphasised. Keith Park discusses the relevance of the circle concept in storytelling and the strong focus, in the poetry workshops, on rhythm. He goes on to evaluate the impact of the workshops, both in terms of staff confidence and through examples of the pupils' responses mapped against the Qualifications and Curriculum Authority (QCA) framework for recognising the attainments of pupils with learning difficulties. The article concludes with some indications of further developments in this exciting and innovative area of practice.

健康作为一种人力资本投资对提高生产力、促进经济发展和个人发展有着重要的作用。它可以提高劳动者的生产效率，刺激教育方面的投资，使人具有更多的时间和能力利用其他资本要素，具有更大的财力（因减少医疗费用）用于其他方面的投资。所以进行人力资本投资时，不可忽视健康这一要素，它也是人力资本形成的重要途径，也对经济增长起着有力的推动作用。     

Ensuring Content‐Area Learning by Secondary Students with Learning Disabilities

Three factors tied with secondary student success in content‐area reading are demonstrated: (a) validated teacher‐focused and student‐focused interventions, (b) integrated and comprehensive service delivery systems, and (c) well‐designed, data‐based professional developmental programs. Difficult challenges face secondary students with LD and their teachers with regard to these students’ participation and success in required general education classes. Recently, instructional methods and materials have been developed and validated for promoting these students’ success. Some of them focus on how general education teachers plan and teach their content‐area courses; others focus on giving students the strategies they need to respond independently to the demands of their courses. This article describes these instructional methods, a service‐delivery model for implementing these interventions in secondary schools, and professional‐development mechanisms and administrative support that must be in place for the model to be maintained effectively.     

Cerebral activity prior to motion task performance: an electroencephalographic study

Cerebral activity, as reflected by power in the alpha waveband (8-13 Hz), was examined prior to the performance of three tasks with different effort and accuracy requirements. The study aimed to further investigate changes in activity observed in an earlier study in which expert karate performers exhibited significant increases in alpha band power during preparation for a training task. In contrast to the earlier research, the high effort task produced a significant decrease in 'alpha' power prior to performance. 'Alpha' power prior to the high accuracy task showed significant differences mediated by the outcome. Successful performance was preceded by higher levels of 'alpha' power than unsuccessful attempts. The reported use of a cognitive strategy on the first task produced a significant difference in activity, lower alpha power being associated with the use of such a technique.
