This study investigated (i) the prevalence of hypohydration and (ii) association between urinary indices of hydration status and confounding factors (e.g., urine protein content, water intake) in elite youth boxers during their weight-stable phase before competition. Sixteen national champion boxers (all male, 17 ± 1 y) were measured on 3 occasions (baseline, day 3, day 10), 30-day prior to competition. Body mass, total body water, urine specific gravity (USG), osmolality (UOSM) and total protein content (TPC) were evaluated to determine hydration status and fluid balance. Overall macronutrient and water intake were assessed using dietary records. Both UOSM and USG increased from day 3 to day 10 by 16% and 0.4% (P < 0.001), despite athletes being in their weight-stability period, and regardless of ad libitum fluid intake. Hypohydration was universally prevalent among all athletes on both test days with USG: 1.027 ± 0.003 g · mL?1 and UOSM: 1035 ± 108 mOsmol · kg?1. An inverse association between mean UOSM values and mean water intake was observed (R = ?0.52, P = 0.04), while TPC was not associated with any urinary dehydration markers (USG, P = 0.51; UOSM, P = 0.61). The present outcomes find that the most prevalent urinary dehydration markers used to classify hydration status in competition exhibit large variability, even during weight-stable periods.  相似文献

Introduction

Postmenopausal women have higher risk of cardiovascular disease. One of the contributing factors could be reduced activity of anti-atherogenic enzyme paraoxonase 1 (PON1). The aim of this study was to examine differences in the lipid status, paraoxonase and arylesterase PON1 activities and PON1 phenotype in women with regular menstrual cycle and in postmenopausal women.

Materials and methods:

The study included 51 women in reproductive age (25 in follicular and 26 in luteal phase of the menstrual cycle) and 23 women in postmenopause. Lipid parameters in sera were determined using original reagents and according to manufacturer protocol. PON1 activity in serum was assessed by spectrophotometric method with substrates: paraoxon and phenylacetate. PON1 phenotype was determined by double substrate method.

Results:

Compared to the women in follicular and luteal phase, postmenopausal women have significantly higher concentration of triglyceride [0.9 (0.7–1.3), 0.7 (0.6–1.0) vs. 1.5 (0.9–1.7) mmol/L; P = 0.002], cholesterol [5.10 (4.78–6.10), 5.05 (4.70–5.40) vs. 6.30 (5.73–7.23) mmol/L; P < 0.001], LDL [3.00 (2.56–3.63), 3.00 (2.70–3.70) vs. 3.90 (3.23–4.50) mmol/L; P < 0.001], and apolipoprotein B [0.88 (0.75–1.00), 0.79 (0.68–1.00) vs. 1.07 (0.90–1.24) mmol/L; P = 0.002]. PON1 basal [104 (66–260), 106 (63–250) vs. 93 (71–165) U/L; P = 0.847] and salt-stimulated paraoxonase activity [210 (131–462), 211 (120–442) vs. 180 (139–296) U/L; P = 0.857] as well as arylesterase activity [74 (63–82), 70 (54–91) vs. 70 (60–81) kU/L; P = 0.906] and PON1 phenotype (P = 0.810) were not different in the study groups.

Conclusion:

There are no differences in PON1 activity and PON1 phenotype between women with regular menstrual cycle and postmenopausal women.  相似文献

IntroductionOesophageal varices are routinely diagnosed by esophagogastroduodenoscopy (EGD), and their bleeding has high mortality. We aimed to evaluate diagnostic performance of biochemical tests in comparison to elastography-based approaches, as non-invasive alternatives to EGD, for ruling-out high risk oesophageal varices (HRV).Material and methodsRetrospective analysis of patients (N = 861) who underwent liver stiffness measurement (LSM) by transient elastography (TE) in a single centre over 5-year period, with available results of EGD (within 3 months from LSM). Only patients with suspicion of compensated advanced chronic liver disease (cACLD) defined by LSM ≥ 10 kPa were included comprising the final cohort of 73 subjects. Original and expanded Baveno VI criteria (B6C), controlled attenuation parameter (CAP), platelet count (PLT), aspartate aminotransferase to PLT ratio index (APRI), Fibrosis-4 index (FIB4), model for end stage liver disease (MELD) score were evaluated against the results of EGD that served as the reference method.ResultsAnalysed patients had median age 62 years, 59/73 (0.81) were males, 54/73 (0.74) had alcoholic/non-alcoholic fatty liver disease, and 21/73 (0.29) had HRV. In multivariate logistic regression analysis only LSM and PLT were independently associated with HRV. The best performing tests for ruling-out HRV (% of spared EGD; % of missed HRV) were respectively: LSM < 20 kPa (53.4%; 0%), B6C (38%; 0%), Expanded B6C (47.9%; 4.8%); PLT > 214x109/L (21.9%; 0%); FIB4 ≤ 1.8 (21.4%; 0%), APRI ≤ 0.34 (12.3%; 0%). CAP, MELD = 6 alone or combined with PLT > 150(x109/L) did not show acceptable performance.ConclusionThe best performing biochemical tests for ruling-out HRV in our cohort of patients were PLT and FIB-4, but they were still outperformed by elastography-based approaches.Keyword: portal hypertension, cirrhosis, platelet count, esophageal varices, non-invasive tests  相似文献

Abstract

In this study, we evaluated the extent to which 2000-m rowing ergometer performance times predicted final rankings at the World Rowing Championships in a sample of 638 rowers of both sexes and body-mass classifications (i.e. open-category and lightweight rowers). Rowing ergometer performance times were examined using a questionnaire, and in 17 of 23 events they were positively correlated (P ≤ 0.049) with the final rankings at the Championships. The highest correlations were for the ergometer performance times achieved by rowers in lightweight men's single sculls (r = 0.78; P = 0.005), women's single sculls (r = 0.75; P = 0.002), men's single sculls (r = 0.72; P = 0.004), and lightweight men's double sculls (r = 0.72; P < 0.001). We used linear regression to establish regression equations to predict final rankings based on 2000-m rowing ergometer performance times for each event in which there was a correlation greater than r = 0.50. Although correlations in 12 events met this criterion, the large standard errors of the estimate hindered ranking predictions in all of the studied events. Regression equations could be used to determine the most probable 2000-m ergometer performance time for a rower to achieve specific rankings at the World Rowing Championships.  相似文献

OBJECTIVES: This paper reports on the training of medical practitioners in information literacy. The course is carried out by an academic medical library in collaboration with its parent institution and the local physicians' licensing body. SETTING: The University of Zagreb Medical School (UZMS) developed an extensive continuing education programme for residents and practitioners, in partnership with the Croatian Chamber of Physicians. Within this programme, the Central Medical Library (CML) offers a hands-on course on searching and appraisal of medical information. DESCRIPTION: Continuing professional development (CPD) helps physicians update and develop the skills and knowledge they require in their everyday practice. The ability to locate, critically evaluate, and use medical information is an important component of CPD. The CML has developed a course entitled "Finding and Appraisal of Medical Information" which has been reviewed and accepted by its parent institution and the physicians' chamber. The course has been introduced in 2005 on a half-year base. OUTCOME: The course provides participants with the information on the most important medical information resources and with the basic skills needed for their effective usage. An evaluation questionnaire is used to assess the effectiveness of the course.  相似文献

Abstract

In this study, we assessed the extent to which 2000-m rowing ergometer performance predicted final rankings at the World Junior Rowing Championship in a sample of 398 junior rowers competing in 13 events. The rowers' ergometer performance times were examined using a questionnaire, and in all 13 events they correlated (P ≤ 0.039) with the final rankings at the Championship. The strongest correlations were observed for ergometer performance times in junior women's single sculls (r = 0.92; P < 0.001), followed by junior men's single sculls (r = 0.80; P < 0.001) and junior women's double sculls (r = 0.79; P < 0.001). The observed correlations were higher for smaller boats – singles, doubles, and pairs (r = 0.64–0.92; P ≤ 0.025) – than for larger boats – quads, fours, and eights (r = 0.31–0.70; P ≤ 0.039). Linear regression analyses were used to construct regression equations to predict final rankings based on 2000-m rowing ergometer performance times for each event. Although correlations in 10 of the 13 events were above r = 0.5, the large standard errors of the estimate impaired the prediction of rankings in all of the studied events. Using these equations, the most probable rowing ergometer performance times required for a particular ranking in a given rowing event might easily be calculated.  相似文献
