经尿道前列腺电切术治疗前列腺增生评价

摘    要:目的评价经尿道前列腺电切术(TURP)治疗前列腺增生症(BPH)的疗效。方法采用德国STORZ电切镜施行经尿道前列腺电切术治疗BPH 2 530例。结果 2 286例获得1年以上随访,最大尿流率由术前6.9±1.2 mL/s上升到术后14.0±2.2 mL/s,残余尿量由术前145.6±33.2 mL下降至28.6±14.2 mL,手术前后有显著差异。术后并发症:暂时性尿失禁196例;肾盂肾炎31例;附睾炎21例;继发性出血32例;会阴水肿5例;残留前列腺组织28例;尿路感染23例;切穿前列腺包膜11例,其中6例出现TUR综合征,死亡2例。结论 TURP是治疗BPH最有效的方法之一,创伤小、出血少、恢复快,但手术操作的技术水平是治疗的难点和关键。

Abstract:Objective To evaluate the effect of transurethral resection of prostate(TURP) to treatment othe benign prostatic hyperplasia(BPH).Methods The transurethral resec-tion BPH 2 530 cases were made up by The implementation of the German STORZ resec-toscope.Results 2 286 patients were followed up for more than one year,the max imum ur-flow rate went up from before surgery 6.9 ± 1.2 mL/s to post-surgery 14.0 ± 2.2 mL,the residual urine volume decreased from preoperative 145.6 ± 33.2 mL/s to post-surgery 28.6 ± 14.2 mL,there are significant difference.The Postoperative complications: 196 cases of incontinence,31cases of Pyelonephritis,21 cases of epididymitis,32 cases of secondary hemorrhage;5 cases of perineal edema,28 cases of residual prostate tissue,23 cases of urinary tract infection,to cut through the pack of prostate was 11 cases;which occurred TUR syndrome in 6 patients and died in 2 cases.Conclusions TURP is one of the most effective method for the treatment of BPH.With less trauma,less bleeding,faster recovery and so on.However,the technical level of operative is the difficulty and criticality for the treatment.
