| 摘 要： | 研究目的:探索符合我国国情的社区高血压患者的延伸性护理模式。研究方法:选取2016年7月至2018年12月间老年高血压患者160人,分为对照组和观察组,每组80人。对照组给予常规护理,观察组采用医院-社区-家庭三元联动的延伸性护理模式。随访6个月后,比较患者治疗的依从性及血压。结果:观察组在患者治疗的依从性及血压控制情况方面,明显优于常规护理组(P<0.05)。结论:医院-社区-家庭三元联动的延伸性护理模式,有助于促进血压的有效控制,值得临床推广和应用。

| Abstract: | Objective:To exploring the community's hypertension patients'extended care model in line with China's national conditions.Methods:160 cases of elderly patients with hypertensive from July 2016 to December 2018 were selected,divided into the study group and the control group,80 cases in each group.The control group received routine care methods,and the study group applied the community-hospital-home care mode.After 6 months of follow-up,the patient's compliance and blood pressure were compared.Results:The compliance and blood pressure control of the study group were significantly better than those of the control group(P<0.05).Conclusion:The community-hospital-home care model promote effective control of blood pressure,which was worthy of clinical promotion and application. |
