萎缩性胆囊炎术式选择

为探讨萎缩性胆囊炎的手术方式，回顾性分析了１９８６－１９９８年经不同手术式方治疗的萎缩性胆囊炎患者８７例，手术并发症以顺序性胆囊切除术最高，为３／２２，胆囊部分切除术后可出现长期上腹隐痛不适，其发生率为２／１８。萎缩性胆囊炎胆囊三角尚能分离者行顺行胆囊切除，难分离者行逆行胆囊切除或胆囊大部切除。

The Modes of Operation for Treatment of Atrophic Cholecystitis

In order to study the modes of operation for atrophic cholecystitis,a retrospective analysis was made for 87 patients with atrophic cholecystitis underwent operations during 1986 1998 and were all cured.There were the highest incidence of operative complications rate of anterograde cholecystectomy and 3 out of 22 patients suffered from it.Long time post operative upper abdominal dull pain happened to 2 out of 18 patients who had undergone partial cholecystectomy.Anterograde cholecystectomy was used for the patients whose calot triangle could easily be distinguished,otherwise retrograde cholecystectomy or partial cholecystectomy should be used instead.Owing to the high cancer incidence of atrophic cholecystitis with cholelithiasis,cholecystectomy and partial hepatectomy was suitable for the patients over the age of 60 when the frozen section shows postive.
